扶正抗痨方联合抗结核治疗对初治肺结核患者免疫功能及IL-1β、IL-1Ra的影响

摘　　要：目的:探讨扶正抗痨方加减联合西医抗结核治疗对初治肺结核患者免疫功能及白细胞介素-1β(IL-1β)、白细胞介素-1Ra(IL-1Ra)表达水平的影响。方法:将初治肺结核89例患者,按随机数字表法分为2组,对照组44例,联合组45例,对照组采用2HREZ/4HR方案,给予抗结核治疗;联合组加用扶正抗痨方加减,2组均连续治疗6个月。观察2组的各项指标变化。结果:治疗前,2组症状评分相比,差异无统计学意义(P>0.05)。治疗后,联合组症状评分(3.58±0.39)分,显著低于对照组(6.14±0.46)分(t=12.389,P<0.05)。治疗后,2组免疫功能指标较前改善,且联合组改善优于对照组(P<0.05)。治疗后,2组IL-1β、IL-1Ra较前改善,且联合组改善优于对照组(P<0.05)。联合组总有效率91.11%,显著高于对照组的77.27%(Z=1.982,P<0.05)。联合组不良反应发生率11.11%,低于对照组29.55%(χ^(2)=4.686,P=0.030<0.05)。结论:扶正抗痨方加减联合西医抗结核治疗初治肺结核,能够通过降低IL-1β、IL-1Ra水平,缓解肺结核的症状,并能够提高患者的免疫能力,疗效确切。Objective:To investigate the effect of modified Fuzheng Kanglao prescription combined with anti-tuberculosis treatment in western medicine on immune function and the expression levels of interleukin-1β(IL-1β)and interleukin-1 Ra(IL-1 Ra)in patients with initial pulmonary tuberculosis.Methods:A total of 89 cases of patients with pulmonary tuberculosis were divided into the control group and the combination group according to the random number table method,44 and45 cases in each group respectively.The control group was given anti-tuberculosis treatment based on 2 HREZ/4 HR regimen,and the combination group was additionally treated with modified Fuzheng Kanglao prescription.Both groups were treated for six months.The changes of various indexes in the two groups were observed.Results:Before treatment,there was no significant difference being found in the comparison of symptom scores between the two groups(P>0.05).After treatment,the symptom score was 3.58±0.39 points in the combination group,significantly lower than that of 6.14±0.46 points in the control group(t=12.389,P<0.05).After treatment,the immune function indexes in the two groups were improved when compared with those before treatment,and the improvement in the combination group was better than that in the control group(P<0.05).After treatment,IL-1βand IL-1 Ra in the two groups were improved when compared with those before treatment,and the improvement in the combination group was better than that in the control group(P<0.05).The total effective rate was 91.11%in the combination group,significantly higher than that of 77.27%in the control group(Z=1.982,P<0.05).The incidence of adverse reactions was 11.11%in the combination group,lower than that of 29.55%in the control group(χ^(2)=4.686,P=0.030<0.05).Conclusion:The therapy of modified Fuzheng Kanglao prescription combined with antituberculosis treatment in western medicine for initial pulmonary tuberculosis can relieve the symptom of pulmonary tuberculosis by reducing the levels of IL-1βand IL-1 Ra,and
