基于“非其时而有其气”湿毒致疫浅谈对新型冠状病毒肺炎的思考

摘　　要："非其时而有其气"是《温疫论》一书提出的当时传染病(疫病)病因,而新型冠状病毒肺炎(COVID-19)具有强烈的传染性,属于中医"疫病"的范畴。从湿毒致病的特点探讨COVID-19的致病机理及防治。湿毒有重浊黏滞缠绵的特点,表现在临床症候群和病程较长等方面;肥人多有阳虚湿盛的体质,感病后易导致疫毒内陷变生诸证而发为重症;防治原则提出应以化湿排毒为主,同时当顾脾胃、护津液;预防上重视防重于治,未病先防,充分发挥中医治未病的特色和方法,从而增强机体免疫力,避免邪毒侵袭,且要因地、因人、因时、因证制宜,把握好扶正与祛邪的关系,更要注重"正气存内,避其毒气"。并指出认识及解决湿毒邪气的问题是COVID-19防治的关键;使中医药在本次防疫治疫中起到应有的作用,发挥中医药防治的优势。Wenyi Lun holds that"abnormal climate"is the etiology of communicable disease(epidemic)at Ming dynasty.Coronavirus disease 2019(COVID-19),with strong infectiousness,belongs to the category of"epidemic"in Chinese medicine.In this paper,the pathogenic mechanism of COVID-19 as well as its prevention and treatment are discussed from the pathogenic characteristics of dampness toxin.Dampness toxin is characterized by heavy turbidity,sticky and lingering,which is manifested in clinical syndrome and a long course of disease,etc.Most obese people have the constitution of yang deficiency and dampness excess;if infected,they are easy to have a variety of syndromes due to the inward invasion of epidemic pathogen and develop into a severe case.The prevention and treatment should focus on resolving dampness and eliminating toxin;meanwhile,it should protect the spleen and stomach as well as the fluid and humor.Besides,we should pay more attention to prevention than treatment and give full play to the characteristics and methods of Chinese medicine in preventing disease before it arises,so as to enhance immunity and avoid the invasion of pathogenic toxin.In addition,we should adjust measures according to the place,person,time and syndrome,and have a good grasp of the relationship between reinforcing the healthy qi and eliminating pathogenic factors;more attention should be paid to"keeping healthy qi inside to prevent the pathogenic toxin".In this paper,it is pointed out that the key to the prevention and treatment of COVID-19 is the understanding of and solutions to dampness toxin and pathogenic qi.It is hoped that Chinese medicine can fully play its role and advantages in the prevention and treatment of the epidemic.
