复方丹参滴丸与硝酸异山梨酯片治疗稳定型心绞痛的Meta分析及药物经济学评价

摘　　要：目的系统评价复方丹参滴丸与硝酸异山梨酯片治疗稳定型心绞痛的疗效,并进行药物经济学分析。方法采用计算机检索中国知网、维普、万方、PubMed、Embase、Cochrane Library等数据库,检索时限为各数据库自建库起至2019年9月1日,提取复方丹参滴丸与硝酸异山梨酯片治疗稳定型心绞痛的随机对照试验,计算复方丹参滴丸组(观察组)与硝酸异山梨酯片组(对照组)症状和心电图改善的有效率;用Review Manager 5.3软件进行Meta分析,并在此基础上采用成本-效果分析法进行经济学评价。结果观察组与对照组症状改善有效率分别为91.21%和76.56%,心电图改善有效率分别为76.27%和52.19%,Meta分析结果显示,观察组上述两指标的改善效果均明显优于对照组[OR=0.31,95%CI(0.23,0.40),P<0.00001],[OR=0.31,95%CI(0.24,0.39),P<0.00001]。药物经济学评价结果显示,增量成本-效果比值为566.73元。敏感性分析结果显示,该评价结果稳定性较好。结论与硝酸异山梨酯片相比,复方丹参滴丸治疗稳定型心绞痛具有良好的有效性和安全性,且有一定的经济优势。临床可根据患者的支付意愿选择治疗方案,若意愿支付值高于566.73元,则复方丹参滴丸具有经济学优势。Objective To systematically review the efficacy of Compound Danshen Dripping Pills and Isosorbide Dinitrate Tablets in the treatment of stable angina pectoris,and to analyze their pharmacoeconomics.Methods CNKI,VIP,WanFang,PubMed,Embase,the Cochrane Library and other databases were searched from the inception to September 1 st,2019 to collect the randomized controlled trials(RCTs)of Compound Danshen Dripping Pills(observation group)and Isosorbide Dinitrate Tablets(control group)in the treatment of stable angina pectoris.The effective rates of symptoms and electrocardio-gram(ECG)of patients with stable angina pectoris in the two groups were calculated.Review Manager 5.3 software was used for Meta-analysis,and cost-effectiveness analysis was used to evaluate the economics of the two drugs.Results The effective rates of symptom improvement in the observation group and the control group were 91.21%and 76.56%,respectively.The effective rates of ECG improvement in the observation group and the control group were76.27%and 52.19%,respectively.Meta-analysis results showed that the improvement effect of the above two indicators in the observation group was significantly better than that in the control group[OR=0.31,95%CI(0.23,0.40),P<0.00001;OR=0.31,95%CI(0.24,0.39),P<0.00001].The results of the pharmacoeconomic evaluation showed that the incremental cost-effectiveness ratio was CNY 566.73.The results of sensitivity analysis showed that the evaluation result was stable.Conclusion Compared with Isosorbide Dinitrate Tablets,Compound Danshen Dripping Pills have good efficacy and safety,and have certain economic advantages.Clinicians can choose the therapy according to the patient’s different willingness to pay(WTP)values.If a patient’s WTP value is higher than CNY566.73,Compound Danshen Drop Pills have economic advantages.
