摘　　要：目的探讨临床药师在特殊患者抗凝治疗管理中的用药监护作用。方法临床药师参与1例颅内静脉血栓形成伴蛛网膜下腔出血的妊娠患者的抗凝治疗,通过药学循证,对患者的常规抗凝方案和围术期抗凝方案提出建议,并于治疗全程实施药学监护。结果医师采纳临床药师建议,患者病情得到有效控制,治疗过程未发生不良反应。结论临床药师参与特殊患者抗凝治疗个体化用药方案的制订和用药监护,可有效保障患者的用药安全性和有效性。Objective To investigate the role of clinical pharmacists in the management of anticoagulant therapy for special patients.Methods The clinical pharmacist participated in the anticoagulant therapy of a pregnant patient with cerebral venous sinus thrombosis(CVST)and subarachnoid hemorrhage,made suggestions on the routine anticoagulant scheme and perioperative anticoagulant scheme through pharmaceutical evidence-based,and implemented pharmaceutical care for the patient in the whole course of the treatment.Results The doctor adopted the clinical pharmacist’s advice,the patient’s condition was effectively controlled,and no adverse reactions occurred during the treatment.Conclusion Clinical pharmacists participate in the formulation and monitoring of individualized anticoagulant therapy for special patients can effectively ensure the safety and effectiveness of anticoagulant therapy.
