四肢开放性骨折伴软组织缺损一期皮瓣修复的临床研究

摘　　要：目的分析对四肢开放性骨折伴软组织缺损患者实施一期皮瓣修复的临床效果。方法择取88例四肢开放性骨折伴软组织缺损患者,将其随机分组研究,对照组(44例)实施常规修复,研究组(44例)实施一期皮瓣修复,对照分析两组临床效果。结果从临床疗效来看,研究组总有效率(100%)高于对照组(88.64%),P<0.05;从住院时间来看,研究组比对照组短,P<0.05;从住院费用来看,研究组比对照组低,P<0.05;从骨折愈合时间来看,研究组(48.77±7.10)d比对照组(66.20±6.48)d短,P<0.05;从关节活动度来看,修复后研究组关节伸直度、背伸度、屈曲度、跖屈度均大于对照组,P<0.05;从并发症情况来看,研究组骨折延迟愈合、功能障碍、皮瓣/皮片部分坏死、骨髓炎等并发症总发生率(6.82%)低于对照组(25.00%),P<0.05。结论对于四肢开放性骨折伴软组织缺损患者,一期皮瓣修复可提高修复效果,缩短住院时间、骨折愈合时间,减少住院费用,改善关节活动度,避免并发症发生,临床价值显著。Objective To analyze the clinical effect of one-stage flap repair in patients with open fracture of limbs and soft tissue defect.Methods 88 cases of limbs open fracture with soft tissue defect were selected and randomly divided into two groups.The control group(44 cases)was treated with routine repair,and the study group(44 cases)was treated with primary flap repair.Results In terms of clinical efficacy,the total effective rate of the study group(100%)was significantly higher than that of the control group(88.64%),P<0.05;from the perspective of hospitalization time,the study group was shorter than the control group,P<0.05;from the perspective of hospitalization expenses,the study group was lower than the control group,P<0.05;from the fracture healing time,the study group(48.77±7.10)d was shorter than the control group(66.20±6.48)d,P<0.05;from the joint activity,the study group was shorter than the control group,P<0.05.In terms of mobility,the joint extension,back extension,flexion and plantar flexion in the study group were greater than those in the control group,P<0.05;from the perspective of complications,the total incidence of delayed fracture healing,dysfunction,partial necrosis of skin flap/skin flap,osteomyelitis in the study group(6.82%)was lower than that in the control group(25.00%),P<0.05.Conclusion For patients with open fracture of limbs with soft tissue defect,one-stage skin flap repair can improve repair effect,shorten hospitalization time,fracture healing time,reduce hospitalization expenses,improve joint mobility,avoid complications,and has significant clinical value.
