TNF-α和TGF-β1基因多态性与关节假体无菌性松动、假体周围骨溶解的相关性分析

摘　　要：目的:讨论分析TNF-α238G/A、TGF-β1+869T/C、+915G/C共3个基因位点的单核苷酸多态性与关节假体无菌性松动、假体周围骨溶解发生风险的相关性。方法:回顾性分析2017年8月1日—2020年12月31日在我院随访到的人工髋关节置换患者共50例,其中关节假体无菌性松动、假体周围骨溶解20例作为观察组,假体牢固患者30例作为对照组。一代测序Sanger法对目的基因片段进行测序,对TNF-α238G/A、TGF-β1+869T/C、+915G/C共3个基因位点进行基因分型。并对各个位点的基因分型与关节假体无菌性松动、假体周围骨溶解的相关性进行比较分析。结果:TNF-α基因238G/A位点:观察组GG基因型18例,AA基因型2例,未检出GA基因型;对照组30例均为GG基因型,未检出AA基因型、GA基因型。由于结果中AA基因型理论频数太小,采用Fisher确切概率法进行统计结果分析,得出P>0.05,提示差异无统计学意义,尚不能认为观察组、对照组TNF-α基因238G/A位点的基因型存在相关性。TGF-β1+869T/C、+915G/C两个基因位点未发现SNP多态性,其中TGF-β1+869T/C位点观察组及对照组均为CC基因型,TGF-β1+915G/C位点观察组及对照组均为GG基因型。结论:目前尚不能认为TNF-α238、TGF-β1+869、+915位点的单核苷酸多态性与关节假体无菌性松动、假体周围骨溶解发病存在相关性,其发病可能潜在易感基因有待进一步深入研究明确。Objective:To discuss and analyze relationship between single nucleotide polymorphisms of TNF-α238G/A,TGF-β1+869T/C and+915G/C with aseptic loosening and periprosthetic osteolysis.Methods:From August 1st 2017 to December 31st 2020,50 patients with hip replacement were followed up in our hospital.Among them,20 cases of joint prosthesis who were aseptic loosening and periprosthetic osteolysis belonged to the observation group,while 30 cases with solid prosthesis belonged to the control group.Target gene fragments were sequenced by first generation Sanger method,and genotyping were classificated between TNF-α238G/A,TGF-β1+869T/C and+915G/C.The correlation was compared between genotyping of each locus and aseptic loosening and periprosthetic osteolysis.Results:TNF-α238G/A gene locus:there were 18 GG genotypes,2 AA genotypes and no GA genotype in observation group;All 30 cases in the control group were GG genotypes,and there were no AA genotypes and GA genotypes.Because the frequency of AA genotype theory was too small,Fisher exact probability method was used to analyze the statistical results.And in result P>0.05,there was no statistically significant difference,so the genotypes of TNF-α238G/A locuscan can not be considered correlation in observation group and control group.There were no SNP polymorphism finded in TGF-β1+869T/C and 915G/C,all the patients were CC genotype in observation group and control group in TGF-β1+869T/C,and all the patients were GG genotype in observation group and control group in TGF-β1+915G/C.Conclusion:The SNP in TNF-α238,TGF-β1+869,+915 were not yet considered to have the relationship between aseptic loosening and periprosthetic osteolysis.The potential susceptibility genes were need to be further studied.
