摘  要:目的:研究破格救心汤加减对心力衰竭模型大鼠的干预作用及改善心肌能量代谢的机制。方法:选取SPF级SD健康大鼠30只,随机选取10只作为空白组,对另外20只大鼠进行心力衰竭造模,建模成功后随机分为模型组和给药组各10只。给药组大鼠用10mL/kg破格救心汤加减灌胃,空白组和模型组用等剂量生理盐水灌胃,连续治疗6周后观察各组大鼠心肌病理变化,检测心功能、血流动力学、心肌能量代谢情况以及心肌组织中沉默信息调节因子2同源蛋白1/过氧化物酶体增殖物激活受体γ辅激活因子-1α(SIRT1/PGC-1α)通路的蛋白表达。结果:模型组大鼠腺嘌呤核糖核苷酸(AMP)、游离脂肪酸(FFA)、AMP/ATP高于空白组,腺苷三磷酸(ATP)含量、Na+-K+-ATP酶、Ca+-ATP酶活性低于空白组,差异有统计学(P<0.05);模型组大鼠心肌组织中SIRT1、PGC-1α蛋白表达量低于空白组,差异有统计学意义(P<0.05);给药组大鼠心肌组织中腺苷三磷酸(ATP)含量、Na+-K+-ATP酶、Ca+-ATP酶活性显著高于模型组,AMP、FFA、AMP/ATP水平显著低于模型组(P<0.05);给药组大鼠心肌组织中SIRT1、PGC-1α蛋白表达明显高于模型组(P<0.05)。结论:破格救心汤加减可在一定程度上改善心力衰竭大鼠心肌能量代谢,促进心功能恢复,其作用机制可能与激活SIRT1/PGC-1α信号通路相关。Objective:To study the effect and mechanism of Decoction for Poge Jiuxin on myocardial energy metabolism of rats with heart failure.Methods:30 healthy SD rats of SPF grade were selected.10 rats were randomly selected as blank group.Another 20 rats were used to establish heart failure model.After successful modeling,they were randomly divided into model group and administration group.The rats in the administration group were treated with 10 mL/kg Poge Jiuxin Decoction,and the rats in the blank group and the model group were treated with the same dose of saline.The pathological changes of the myocardial tissue were observed after 6 weeks of continuous treatment.Cardiac function,hemodynamics,myocardial energy metabolism and expression of silence information regulator 2 homologous protein 1/peroxisome proliferatoractivated receptorγcoactivator-1α(SIRT1/PGC-1α)pathway in myocardium were measured.Results:Adenine ribonucleotide(AMP),free fatty acid(FFA),AMP/ATP were higher in the model group than in the blank group,and adenosine triphosphate(ATP)content,Na+-K+-ATPase and Ca+-ATPase activities were lower in the model group than in the blank group,with statistically significant differences(P<0.05).The expression levels of SIRT1 and PGC-1 in the myocardial tissue of the model group were lower than that of the blank group,with statistically significant differences(P<0.05).The levels of adenosine triphosphate(ATP),Na+-K+-ATPase and Ca+-ATPase in the myocardial tissue of rats in the treatment group were significantly higher than those in the model group,and the levels of AMP,FFA and AMP/ATP were significantly lower than those in the model group(P<0.05).The expression levels of SIRT1 and PGC-1 in rat myocardium were significantly higher than those in the model group(P<0.05).Conclusion:The addition and reduction of poge Xiexin decoction can improve myocardial energy metabolism and promote cardiac function recovery in rats with heart failure,and the mechanism may be related to the activation of SIRT1/PGC-1 signaling pathway.
