后路短节段椎弓根螺钉固定联合椎体成形术治疗胸腰椎Magerl A3型骨质疏松性骨折的临床效果

摘　　要：目的探究后路短节段椎弓根螺钉固定联合椎体成形术治疗胸腰椎Magerl A3型骨质疏松性骨折的临床效果。方法将2016年1月至2019年12月收治的68例胸腰椎Magerl A3型骨质疏松性骨折患者采用简单随机化法分为对照组和试验组,每组34例。对照组实施后路短节段椎弓根螺钉固定与伤椎置钉联合治疗,试验组实施后路短节段椎弓根螺钉固定与椎体成形术联合治疗。比较两组的治疗效果。结果试验组的手术出血量少于对照组,手术用时、术后下床活动时间、住院时间均短于对照组(P<0.05)。术后,试验组的伤椎前缘高度、伤椎后缘高度高于对照组,VAS评分低于对照组(P<0.05)。试验组的腰椎功能优良率高于对照组,并发症总发生率低于对照组(P<0.05)。结论后路短节段椎弓根螺钉固定联合椎体成形术治疗胸腰椎Magerl A3型骨质疏松性骨折,可更好改善患者的腰椎功能,降低疼痛程度,且可缩短治疗和康复的时间,降低并发症发生率,对患者预后的改善有积极意义。Objective To explore the clinical effect of posterior short segment pedicle screw fixation combined with vertebroplasty in the treatment of thoracolumbar Magerl A3 osteoporotic fracture.Methods From January 2016 to December 2019,68 patients with thoracolumbar Magerl A3 osteoporotic fracture were divided into control group and experimental group according to simple randomized method,with 34 cases in each group.The control group was treated with posterior short segment pedicle screw fixation combined with vertebral pedicle screw placement,and the experimental group was treated with posterior short segment pedicle screw fixation combined with vertebroplasty.The treatment effects of the two groups were compared.Results The operative blood loss of the experimental group was less than that of the control group,and the operation time,postoperative ambulation time and hospital stay were shorter than those of the control group(P<0.05).After operation,the height of anterior edge and posterior edge of injured vertebra in the experimental group were higher than those in the control group,and the VAS score was lower than that in the control group(P<0.05).The excellent and good rate of lumbar function in the experimental group was higher than that in the control group,and the total incidence of complications was lower than that in the control group(P<0.05).Conclusion The posterior short segment pedicle screw fixation combined with vertebroplasty in the treatment of thoracolumbar Magerl A3 osteoporotic fracture can better improve the lumbar function of patients,reduce the degree of pain,shorten the time of treatment and rehabilitation,and reduce the incidence of complications,which has a positive significance for the improvement of the prognosis of patients.
