摘  要:目的:探究糖尿病人群经桡动脉冠状动脉造影术后桡动脉损伤的危险因素,为糖尿病人群介入路径的保护及重复利用提供理论依据。方法:入选2015年9月至2019年4月期间,于我院首次行冠状动脉造影检查并明确诊断2型糖尿病的631例患者。对其进行术前及术后桡动脉超声数据的采集。根据术后桡动脉闭塞与否将患者分为闭塞组与非闭塞组,多个模型对总体人群进行Logistic多因素分析。ROC曲线分析模型的预测价值。结果:术后共有58例(9.2%)例出现桡动脉闭塞。多个模型Logistic多因素分析提示HbA1c是糖尿病患者桡动脉闭塞的危险因素(OR=1.532,95%CI:1.261-1.860,P<0.001)。HbA1c对糖尿病患者的桡动脉闭塞具有很好的预测价值。结论:糖尿病患者血糖管理水平影响着介入路径的损伤修复,增加桡动脉闭塞事件的发生。Objective:Patients with diabetes complicated with coronary heart disease often need multiple coronary angiography and interventional therapy,and the injury of interventional pathway is far more than that of non-diabetic patients,so how to predict the injury of interventional pathway becomes of great significance.The purpose of this study is to explore the risk factors of radial artery injury in diabetic patients after radial coronary angiography,and to provide theoretical basis for the protection and reuse of interventional pathway in diabetic patients.Methods:From September 2018 to April 2019,631 patients who underwent coronary angiography for the first time and were diagnosed with type 2 diabetes mellitus were selected.Ultrasound data of radial artery were collected before and after operation.Patients were divided into occlusion group and non-occlusion group according to whether the radial artery was occluded or not after operation.Logistic multivariate analysis was performed on the whole population by multiple models.Predictive value of ROC curve analysis model.Results:58 cases(9.2%)had radial artery occlusion after operation.Logistic multivariate analysis showed that glycosylated hemoglobin was an independent risk factor for radial artery occlusion in diabetic patients(OR-1.532,95%CI:1.261-1.860,P<0.001).Glycosylated hemoglobin has a good predictive value for radial artery occlusion in diabetic patients.Conclusions:The level of blood glucose management in diabetic patients affects the injury repair of interventional pathway and increases the incidence of radial artery occlusion.
