重症医学科心外术后肺部感染危险因素分析

摘　　要：目的:分析综合重症监护病房(GICU)心脏外科术后发生肺部感染的危险因素,以针对性的提出干预措施。方法:回顾性分析2018年1月至2020年1月,重症医学科2714例心脏术后,根据临床表现、胸X线片及细菌学结果,确诊术后发生肺部感染。对患者肺部感染可能相关因素进行多因素Logistic回归分析。结果:59(2.2%)例患者发生肺部感染。相较于无肺部感染组,肺部感染患者年龄大,手术时间长,呼吸机应用时间及GICU住院时间长(P均<0.05)。多因素Logistic回归分析显示术前住院天数,呼吸机使用时间,ICU天数,手术时间等因素均是心外术后发生肺部感染的危险因素(P<0.05)。结论:心外术后肺部感染与多种因素有关,加强肺部感染控制,可以降低术后并发症,提高临床疗效。Objective:Study and analysis the risk factors for postoperative pulmonary infection in patients under going cardiac sugery.Methods:From January 2018 to January 2020,2714 cases of heart surgery in the Department of Critical Care Medicine were retrospectively analyzed.According to clinical manifestations,chest radiographs and bacteriological result,the postoperative pulmonary infection was confirmed.Multivariate Logistic regression analysis was carried out on the possible related factors of pulmonary infection in patients.Results:Lung infection occurred in 59 patients(2.2%).Compared with the group without pulmonary infection,the patients with pulmonary infection had older age,longer operation time,longer ventilator application time and longer hospitalization time in ICU(P<0.05).Multivariate Logistic regression analysis showed that preoperative hospitalization days,breathing machine use time,ICU days,operation time and other factors were independent risk factors for pulmonary infection after cardiac surgery(P<0.05).Conclusions:Patients with pulmonary infection after cardiac sugery is related to multiple factors,strengthen lung infection control,may reduce the incidence of infection underwent cardiac sugery,in order to improve clinical efficacy.
