专科护士主导的老年肠造口MDT个案管理模式的应用研究

摘　　要：目的探讨专科护士主导的老年肠造口MDT个案管理模式对患者居家康复阶段的自我护理能力及自我感受负担的影响。方法选取2017年1月1日至2018年8月31日在该院行肠造口术的老年患者76例,随机分为对照组和干预组各38例,对照组使用常规护理模式进行护理,干预组在此基础上实行专科护士主导的MDT个案管理模式进行干预。分别于术后3个月、6个月比较两组患者的自我护理能力和自我感受负担差异。结果术后6个月与术后3个月比较,干预组的自我护理能力明显提升,差异有统计学意义(p<0.001);干预前后比较,干预组自我感受负担得到明显改善,差异均有统计学意义(均P<0.001);干预后不同阶段,干预组的自我护理能力均高于对照组,自我感受负担均低于对照组,差异均有统计学意义(均P<0.001)。结论将专科护士主导的MDT个案管理模式应用于老年肠造口患者的护理,能明显提升患者在居家康复阶段的自我护理能力,降低自我感受负担。Objective To explore the influence of specialized nurse led multidisciplinary team(MDT)case management mode of enterostomy on the self-care ability and self-perceived burden of the elderly patients in the stage of rehabilitation at home.Methods A total of 76 elderly patients receiving enterostomy in this hospital on January 1,2017 and August 31,2018 were selected and randomly divided into control group and intervention group,38 cases in each group.The control group was treated with conventional nursing mode,and the intervention group was treated with MDT case management mode led by specialized nurses on this basis.The self-care ability and self-perceived burden of patients in the two groups were compared at 3 and 6 months after surgery.Results The self-care ability of the intervention group was significantly improved 6 months after operation compared with 3 months after operation,and the difference was statistically significant(P<0.001).Compared with before intervention,the self-perceived burden of the intervention group was significantly improved after intervention,and the difference was statistically significant(P<0.001).At different stages after intervention,the self-care ability of the intervention group was higher than that of the control group,and the self-perceived burden of the intervention group was lower than that of the control group,and the differences were statistically significant(P<0.001).Conclusions Applying the MDT case management mode led by specialized nurses to the nursing of elderly patients with enterostomy can significantly improve the patientsx self-nursing ability in the rehabilitation stage at home and reduce the burden of self-feeling.
