摘  要:目的探讨CD64、CD11b及淋巴细胞亚群在不同类型感染性疾病中的表达差异。方法选取细菌感染患者40例(细菌组),包括20例全血感染(全血组),20例局部感染(局部组);EB病毒感染患者20例(病毒组);同期健康体检者20例(对照组),采用流式细胞术检测其外周血中的中性粒细胞CD64、CD11b及淋巴细胞亚群的表达强度,并对检测结果进行分析。结果细菌组的CD64指数、B细胞表达显著高于病毒组和对照组(P<0.05),而中性粒细胞和单核细胞CD11b平均荧光强度显著高于对照组(P<0.05),但低于病毒组。病毒组CD3+T细胞比率较细菌组显著升高(P<0.05),CD8+T细胞比率较另外两组均显著升高(P<0.05);细菌组的双阴性T(DN-T)细胞比率显著低于病毒组和对照组(P<0.05);细菌组和病毒组的NK细胞比率与对照组相比均有下降的趋势,但无统计学的差异(P>0.05);双阳性T(DP-T)细胞比率三组之间比较无显著差异(P>0.05);在细菌组,全身感染组与局部感染组之间以上指标无统计学差异(P>0.05)。结论测定外周血中性粒细胞CD64的表达对诊断细菌性的感染具有重要意义。外周血中性粒细胞和单核细胞CD11b的表达在感染发生时可升高,对于病毒性感染升高更明显。联合检测淋巴细胞亚群,能评估患者的免疫功能状态,为临床感染患者的治疗提供可靠的参考和依据。Objective To analyze the expression of CD64, CD11 b and lymphocyte subsets in peripheral blood from different kinds of infectious diseases. Methods 40 cases of bacterial infection(bacterial group) including 20 cases of whole blood infection and 20 cases of local infection, 20 patients infected with Epstein-Barr virus(virus group) and 20 healthy subjects(control group) were selected. Flow cytometry was used to detect the lymphocyte subsets and the expression of CD64、 CD11 b in peripheral blood. Results The expression of CD64 and the ratio of B cells in the bacterial group were significantly higher than that in the virus group and control group(P<0.05). The expression of CD11 b in neutrophiles and monocytes and the ratio of CD3+T cells were increased in virus group by comparing with bacterial group(P<0.05), and the expression of CD11 b in these groups were higher than that in control group. The ratio of CD8+T cells in the virus group was much higher than that in the other two groups(P<0.05). The DN-T cells in bacterial group was much lower than that in virus group and control group(P<0.05).Two groups of The ratio of NK cells was reduced in virus and bacterial groups to some extent and was lower than the control group,but the difference was not significant(P>0.05).The ratio of DP-T cell among the groups had no significant difference(P>0.05). In the bacterial group, there was no statistical difference in the above indexes between the systemic infection group and the local infection group(P>0.05). Conclusions The detection of the expression of CD64 in neutrophile is meaningful in the diagnosis of bacterial infection. The expression of CD11 b in neutrophiles and monocytes can be increased during infection, and the increase is more obvious for viral infection. Combined detection of lymphocyte subsets can evaluate the status of immune function of patients, and provide reliable reference and basis for the treatment of patients with clinical infection.
