波动式负压封闭引流技术中不同吸引压力对中华眼镜蛇咬伤的治疗效果观察

摘　　要：目的:探讨波动式负压封闭引流技术中不同吸引压力对中华眼镜蛇咬伤患者的临床治疗效果。方法:选取2018年1月-2020年1月在我院急诊科就诊的中华眼镜蛇咬伤患者120例,根据数字表法随机分为对照组和观察组,每组60例。对照组在早期程序化综合治疗基础上采用波动式负压封闭引流技术(350mmHg);观察组在此基础上维持负压400mmHg。比较两组患者皮肤组织坏死积分、相关指标、治疗有效率。结果:治疗后5d,两组患者皮肤组织坏死积分均低于治疗前,差异有统计学意义(P<0.05),且观察组积分低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后5d各项指标均下降,观察组CK、LDH、hs-CRP水平均低于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率93.33%,高于对照组的85.00%,但差异无统计学意义(P>0.05)。结论:波动式负压封闭引流技术在中华眼镜蛇咬伤患者的治疗中负压调节400mmHg临床效果最佳,能有效改善患者皮肤组织坏死,降低ck、LDH和hs-CRP水平。Objective To investigate the clinical effects of different suction pressures in the undulating negative pressure sealing drainage technology on Chinese cobra bite patients.Methods A total of 120 Chinese cobra bite patients admitted to the emergency department of our hospital from January 2018 to January 2020 were selected and randomly divided into control group and observation group according to number table method,with 60 cases in each group.The control group was treated with the technique of undulating negative pressure closed drainage(350 mmHg)on the basis of early programmed comprehensive treatment.The observation group maintained a negative pressure of 400 mmHg on this basis.Skin tissue necrosis score,related indexes and treatment effective rate were compared between 2 groups.Results On day 5 after treatment,the scores of skin tissue necrosis in 2 groups were lower than before intervention(P<0.05),and the scores of observation group were lower than control group,the difference was statistically significant(P<0.05).All indicators of 2 groups decreased 5 days after treatment,CK,LDH and hs-CRP levels in the observation group were lower than those in the control group,with statistical significance(P<0.05).The effective rate of the observation group was 93.33%,which was higher than that of the control group(85.00%),and the difference was statistically significant(P<0.05).Conclusion The undulating negative pressure sealing drainage technique has the best clinical effect in the treatment of patients with Chinese cobra bite by negative pressure adjustment of 400 mmHg,which can effectively improve the skin tissue necrosis of patients and improve the therapeutic effect.
