摘 要:目的:对比早产儿呼吸窘迫综合征患者,采用猪肺表面活性物质与牛肺表面活性物质的治疗效果。方法:选2019年9月-2020年9月80例呼吸窘迫综合征早产患儿,按照治疗方式不同将其分为观察组和对照组,其中,观察组40例,采用机械通气治疗配合猪肺表面活性物质,对照组40例,采用机械通气治疗配合牛肺表面活性物质,对比两组患儿治疗前后的血气指标、临床相关指标以及并发症发生情况。结果:治疗后观察组患儿的PaO_(2)指标(91.68±6.45)mmHg,pH指标7.44±0.13,PaCO_(2)指标(40.84±13.37)mmHg,均与对照组无显著差异(t_(PaO2)=0.578、tpH=1.324、t_(PaCO2)0.231,PPaO2=0.565、PpH=0.189、P_(PaCO2)=0.818);两组患儿的住院时间、用氧时间对比无显著差异,但是通气时间观察组患儿为(4.12±0.78)d,显著少于对照组患儿的(5.39±0.64)d(t=7.961,P<0.001);两组患儿的并发症总发生率对比无显著差异(χ^(2)=0.292,P=0.704)。结论:对于早产儿呼吸窘迫综合征的临床治疗,采用猪肺表面活性物质与牛肺表面活性物质均有显著效果,且均较为安全,不会有严重的并发症发生。Objective To explore the application effects of porcine lung surfactant and bovine lung surfactant in the clinical treatment of respiratory distress syndrome in preterm infants.Methods From September 2019 to September 2020,80 newborn children with respiratory distress syndrome were selected as the research objects.They were divided into observation group and control group according to different treatment methods.Among them,40 cases in the observation group were treated with mechanical ventilation combined with porcine lung surfactant,40 cases in the control group were treated with mechanical ventilation combined with bovine lung surfactant,and then the blood gas indexes,clinical related indexes and complications of the two groups of children before and after treatment were compared.Results After treatment,the PaO_(2) index(91.68±6.45)mmHg,pH index(7.44±0.13)mmHg,and PaCO_(2) index(40.84±13.37)mmHg of the observation group were not significantly different from those of the control group(t_(PaO2)=0.578,t pH=1.324,t PaCO_(2)=0.231,P_(PaO2)=0.565,PpH=0.189,P_(PaCO2)=0.818);There was no significant difference in hospitalization time and oxygen use time between the two groups,but the ventilation time of the observation group was(4.12±0.78)days,Significantly less than(5.39±0.64)days in the control group(t=7.961,P<0.001);There was no difference in the total incidence of complications between the two groups(χ^(2)=0.292,P=0.704).Conclusion For the clinical treatment of respiratory distress syndrome in premature infants,the use of porcine lung surfactant and bovine lung surfactant have significant effects,and both are relatively safe,and there will be no serious complications.
