温针灸联合整脊法对颈性眩晕患者血清TC、CGRP及椎-基底动脉血流速度的影响

摘　　要：目的:探讨温针灸联合整脊法对颈性眩晕患者血清TC、CGRP及椎-基底动脉血流速度的影响。方法:以2018年1月—2019年12月安徽中医药大学第二附属医院收治的颈性眩晕患者为研究对象,共80例,通过随机数字表法分为试验组、对照组,各40例。两组均给予常规西医治疗,对照组在此基础上采用整脊法进行治疗,试验组在对照组的基础上采用温针灸进行治疗,均治疗4周。比较两组治疗4周后的临床疗效,治疗前、治疗4周后的眩晕症状、颈椎功能恢复情况、椎-基底动脉血流速度及血清细胞因子水平。结果:治疗4周后,试验组总有效率(97.50%,39/40)高于对照组(80.00%,32/40)(P<0.05)。与治疗前相比,治疗4周后,两组眩晕症状评分、LVA、RVA、BA和血清CGRP水平均升高,且试验组均高于对照组(P<0.05);而两组FS-CSA评分、血清D-D、TC水平均降低,且试验组均低于对照组(P<0.05)。结论:温针灸结合整脊法治疗颈性眩晕,可通过降低血清TC水平、提高血清CGRP水平及椎-基底动脉血流速度,改善患者眩晕症状及颈椎功能受损情况,从而提高疗效。Objective:To investigate the effect of needle-warming moxibustion combined with chiropractic therapy on the serum levels of TC and CGRP,as well as on vertebro-basal artery blood flow velocity in patients with cervical vertigo.Methods:80 patients with cervical vertigo were randomly divided into the observation group and the control group,with 40 cases in each group.Both groups were given conventional western medicine treatment,the control group was treated with chiropractic therapy;on which basis,the observation group was also treated with needle-warming moxibustion.After four weeks of treatment,the clinical efficacy was compared between the two groups;the vertigo symptoms,cervical function recovery,vertebral-basal artery blood flow velocity and the serum levels of cytokine were observed and compared between before and after the treatment in the two groups.Results:The total effective rate was 97.50%(39/40)in the observation group,which was significantly higher than 80.00%(32/40)in the control group(P<0.05).The scores of dizziness symptoms,the levels of LVA,RVA,BA and serum CGRP were significantly increased after the treatment compared to those before the treatment in the two groups,and the increases in the observation group were more significant than those in the control group(P<0.05).The scores of FS-CSA,and the serum levels of D-D and TC were decreased in the two groups,the decreases in the observation group were more significant(P<0.05).Conclusion:Needle-warming moxibustion combined with chiropractic therapy can improve the curative effect by reducing the serum level of TC,increasing the serum level of CGRP and the vertebral-basal artery blood flow velocity,improving the patient's vertigo symptoms and impaired cervical function in the treatment of cervical vertigo.
