抑郁症伴失眠德国患者120例中医证候分布特征

摘　　要：目的:探讨抑郁症伴失眠德国患者的中医证候分布、辨证分型规律及各证型失眠的特点。方法:收集2018年11月至2019年9月在北京中医药大学德国魁茨汀医院住院治疗的抑郁症伴失眠患者病例120例。采集其年龄、性别、职业、病史、症状、舌脉象等基本信息,以频数分析、聚类分析、单样本卡方检验等方法分析其中医证候分布规律及证候特点。结果:抑郁症伴失眠患者的常见症状有30项,出现频率较高的前5项依次为忧思善虑、神疲乏力、早醒、兴趣索然及悲伤欲哭;舌、脉象以舌质暗、胖、有瘀斑,舌苔薄白、薄黄、白腻,脉弦细、细滑和弦滑多见;常见辨证分型有:肝郁肾虚证、肝郁气滞证、肝郁脾虚证、肝郁化火证、心脾两虚证和肝胃不和证;各证型失眠以早醒和入睡困难为主。结论:抑郁症伴失眠德国患者常见的中医证型有6类,且不同中医证型失眠的构成情况亦各有特点。Objective:To explore the distribution characteristics of traditional Chinese medicine(TCM)syndromes,the rules of syndrome type of TCM,and characteristics of insomnia of different syndromes in German depression patients with insomnia.Methods:A total of 120 cases of German depression patients with insomnia were collected from November 2018 to September 2019 in the hospital of TCM-Klinik,Bad K?tzting,Germany,Beijing University of Chinese Medicine.And the basic information of age,gender,occupation,history,symptoms,tongue and pulse were collected.Frequency analysis,cluster analysis and single sample chi square test and other methods were used to analyze these data,so as to obtain the distribution law and characteristics of TCM syndrome.Results:There were 30 common symptoms in patients with depression and insomnia,and the top 5 symptoms with high frequency were anxiety,fatigue,early awakening,lack of interest and sad crying.The common tongue is dark,fat,with ecchymosis and the common tongue coating is thin white,thin yellow and white greasy.The common pulse is stringiness,slenderness and string-smoothness.There are six common syndrome types:liver depression and kidney deficiency syndrome,liver depression and qi stagnation syndrome,liver depression and spleen deficiency syndrome,liver depression transforming fire syndrome,heart and spleen deficiency syndrome and liver stomach disharmony syndrome.The main types of insomnia were early awakening and difficulty in falling asleep.Conclusion:There are six common TCM syndromes in Germany depression patients with insomnia,and there are different characteristics in the constitution of insomnia in different syndromes.
