正常青年人左心室扭转特征的MRI研究

摘　　要：目的用特征追踪技术测量正常青年志愿者心脏MRI电影成像上心肌扭转参数,分析左心室心肌扭转运动的规律及扭转参数的参考值范围。资料与方法采集29例24~34岁健康志愿者的心脏基底、中部和心尖3个短轴层面电影成像序列。采用心肌特征追踪软件TOMTEC对MRI电影图像进行分析,获得3个层面心肌旋转角度、旋转速率、扭转和扭矩等参数,分析基底、中部和心尖3个水平心肌扭转参数的分布特征。结果自基底-中部-心尖心肌旋转角度依次增大,达峰时间依次增加。心尖旋转呈单相,逆时针扭转后解旋;心底旋转呈双相,逆时针扭转-解旋-顺时针扭转-解旋。旋转角度和达峰时间个体差异比较大。心尖-心底心内膜和心外膜的最大扭角分别为(13.44±7.18)°、(8.88±4.53)°;心尖-心底心内膜和心外膜的最大扭矩分别为(1.84±0.99)°/cm、(1.21±0.63)°/cm。结论心肌扭转运动有一定的特征性,左心室心尖-心底的扭矩可以是相对恒定的功能参数。Purpose To investigate the patterns of left ventricle myocardial rotation and torsion of the normal young adult volunteers using feature tracking technology(FT)on cardiac MR cine imaging and to setup the reference range for myocardial function evaluation.Materials and Methods Twenty-nine normal youth volunteers aged from 24-34 years old were enrolled in this study.The Cine MR imaging were acquired using balance FFE sequence with 24 phases for each cardiac cycle.The standard short-axis imaging planes included basal,mid-cavity and apical levels.The rotation angle,twist angle between apical and basal level and its torque were calculated and analyzed using feature tracking techniques.Results Both the rotation angles and the time-to-peak of the myocardium from the base to the middle to the apex increased in step.Apical rotation was monophasic,initially anticlockwise rotated and then untwisted to the start position.The basal rotation was biphasic,initially anticlockwise rotated and then untwisted and continued to clockwise rotation.Visually,the myocardial rotated in a regular pattern but the rotation angles and the time to peak varied individually.The maximum twist between apical and basal level of endocardium and epicardium were(13.44±7.18)°,(8.88±4.53)°,respectively.The maximum torque between apical and basal level of endocardium and epicardium were(1.84±0.99)°/cm and(1.21±0.63)°/cm,respectively.Conclusion The left ventricle rotation and its torque showed regular pattern visually but varied in numbers.The torsion of left ventricular between apex and base can be a relatively constant for functional evaluation.
