摘  要:目的通过磁共振T2 mapping和磁共振软骨延迟增强成像(dGEMRIC)评估膝关节再生软骨的修复过程。资料与方法对13例患者的20处再生软骨在基质诱导的自体软骨细胞移植术(MACI)后1、3、6、12个月进行磁共振T2 mapping和dGEMRIC检查,测量再生软骨和邻近正常软骨的T2值和纵向弛豫率差(ΔR1)值,比较术后各时间点再生软骨和正常软骨T2值和ΔR1值的差异,观察再生软骨T2值和ΔR1值在术后1、3、6、12个月的动态变化。结果再生软骨区T2值和ΔR1值在MACI术后1、3、6、12个月呈逐渐下降趋势,且差异有统计学意义(P<0.01)。MACI术后1、3、6个月,再生软骨T2值和ΔR1值高于正常软骨(P均<0.01)。术后12个月,再生软骨的T2值和ΔR1值与正常软骨比较,差异无统计学意义(P均>0.05)。结论磁共振T2 mapping和dGEMRIC能作为评价再生软骨生物特性的有效方法,为临床软骨移植工作的疗效评价提供依据。Purpose To evaluate the maturation of regenerative cartilage of knee joint via MR T2 mapping and delayed gadolinium enhanced MRI of the cartilage(dGEMRIC)technique.Material and Methods A total of 20 regenerative cartilage tissues from 13 patients underwent MR T2 mapping and dGEMRIC examination at 1,3,6 and 12 months after matrix-associated autologous chondrocyte implantation(MACI).The mean T2 values andΔR1 values of regenerative cartilage and control cartilage were measured and calculated.The T2 values andΔR1 values between regenerative cartilage and control cartilage were compared,and the dynamic changes in T2 values andΔR1 values of regenerative cartilage at 1,3,6 and 12 months after MACI were analyzed.Results The mean T2 values andΔR1 values of regenerative cartilage presented a downward trend and there were significant differences in T2 values andΔR1 values at 1,3,6 and 12 months after MACI(P<0.01).The mean T2 values andΔR1 values of the regenerative cartilage were all significantly higher than those of control cartilage at 1,3 and 6 months(P<0.01),but not at 12 months(P>0.05).Conclusion T2 mapping and dGEMRIC techniques own the capability of evaluating the biochemical characteristics of regenerative cartilage,which can be recommended as potential methods to analyze the efficacy of cartilage implantation.
