微创治疗胫骨髁间嵴撕脱骨折的效果观察

摘　　要：目的研究关节镜辅助下不经过骨骺及骺板微创治疗青少年胫骨髁间嵴撕脱骨折的临床效果。方法将30例骨骺未闭合的胫骨髁间嵴撕脱骨折患者随机分为两组,观察组(关节镜)18例,对照组(传统切开)12例,观察组取关节镜辅助下钢丝固定并经皮下固定于胫骨,对照组行空心钉固定。对比两组手术时间、住院时间、康复时间、住院费用,膝关节功能及膝关节活动度。结果两组手术时间、住院时间、康复时间及住院费用比较,差异有统计学意义(P<0.05);两组术后6周Lysholm评分及膝关节活动度比较,差异有统计学意义(P<0.05),末次随访时比较,差异无统计学意义(P>0.05)。结论关节镜微创辅助不经过骨骺及骺板的钢丝固定治疗骨骺未闭合的胫骨髁间嵴撕脱骨折具有不损伤骺板及骨骺、创伤小、恢复快、膝关节功能恢复好、花费少等优点。Objective To explore the clinical effect of arthroscopy assisted minimally invasive treatment of adolescent tibial intercondylar ridge avulsion fracture without going through the epiphysis and epiphyseal plate.Methods 30 patients with unclosed tibial intercondylar ridge avulsion fractures were randomly divided into two groups.18 cases in the observation group got arthroscopy assistant treatment,and 12 cases in the control group got traditional incision.The observation group gotsteel wire fixation subcutaneously to tibia assisted by arthroscopy,while the control group was fixed with hollow nails.The operation time,hospitalization time,rehabilitation time,hospitalization expenses,knee function and motion range of knee were compared between the two groups.Results There was statistically significant difference in operation time,hospitalization time,recovery time and hospitalization expenses between the two groups(P<0.05).There was statistically significant difference in Lysholm score and knee joint mobility between the two groups 6 weeks after operation(P<0.05);the difference was not statistically significant at the last follow-up(P>0.05).Conclusion Arthroscopyically assisted minimally invasive treatment of adolescent tibial intercondylar ridge avulsion fracture has the advantages of little trauma,quick recovery,good knee function recovery,low cost,etc.
