混合管理模式的眼科日间手术流程

摘　　要：目的总结上海交通大学医学院附属仁济医院在以集中式管理模式为主的混合式日间手术管理模式下的眼科日间手术管理流程。方法对2018年1-12月在该院完成的3533例眼科日间手术,从入院前管理、入院后术前准备、术中管理、术后管理和出院管理几个环节进行流程优化管理。结果在手术期间,所有入选病例均未出现术后并发症和不良事件,手术室流程良好,无术后感染病例。局部麻醉患者术后在病房观察护理2~4 h,顺利出院;全身麻醉患者观察护理至次日检查换药后出院。出院患者的满意度为98.62%。结论眼科日间手术采取集中管理为主的混合式管理模式,能充分利用资源,专科化管理值得临床推广。Objective To summarize the management procedures of daytime surgery in the Ophthalmology Department of the hospital under the mixed day surgery management mode dominated by the centralized management mode.Methods Optimized procedural management was made in the 3533 cases of ophthalmic operations completed in the hospital from January to December 2018,through the procedures of pre-admission management,pre-admission preparation,intra-operative management,post-operative management as well as management after discharge.Results During surgery,no adverse events and postoperative complications occurred in any patients.The operating room worked in good order,and no case of infection was found in all the patients.All the patients with local anesthesia were discharged from hospital without any event after observation and nursing in the ward for 2-4 hours after surgery,and those with general anesthesia were also discharged from hospital after they were observed and nursed until the next day.The satisfaction rate of the patients was 99.88%.Conclusion Daytime surgery with a mixed management mode based on the"centralized management"has the advantage of making full use of medical resources,and its specialized management could ensure the safety of patients.For this reason,it is worth further clinical extension.
