全身振动疗法结合运动疗法治疗脑卒中后倾斜综合征的疗效

摘　　要：目的探讨全身振动疗法结合运动疗法治疗脑卒中后倾斜综合征的临床疗效。方法42例脑卒中后倾斜综合征患者随机分为观察组和对照组,每组21例。在常规治疗的基础上,观察组在矫正镜前端坐位下进行全身振动治疗20 min,对照组在矫正镜子前端坐练习20 min,之后均接受常规运动疗法30 min。每周治疗5次,连续4周。在治疗前和治疗4周后评估疗效相关指标。结果两组治疗后Burke倾斜量表评分、睁眼和闭眼X轴平均压力中心绝对值、睁眼和闭眼运动椭圆面积小于治疗前,Berg平衡量表评分大于治疗前(P<0.01)。观察组治疗后Burke倾斜量表评分、睁眼和闭眼X轴平均压力中心绝对值、睁眼和闭眼运动椭圆面积小于对照组,Berg平衡量表评分大于对照组(P<0.05)。在治疗期间和治疗结束后1周均无不良反应发生。结论全身振动疗法结合运动疗法可以有效提高脑卒中后倾斜综合征患者平衡能力,改善倾斜症状。Objective To observe the efficacy of whole body vibration therapy combined with exercise therapy in the treatment of patients with pusher syndrome after stroke.Methods Fourty-two patients with pusher syndrome after stroke were divided into two groups with 21 cases each.On the conventional treatments, group A was treated additionally with whole body vibration therapy sitting in front of a mirror for 20 minutes, which was foolowed by routine exercise therapy for 30 minutes.Group B was treated with routine exercise therapy for 30 minutes.The treatment was performed five times a week for 4 weeks.The efficacy-related indicators were evaluated before and 4 weeks after treatment.Results The scores of Burke inclination scale and absolute value of mean pressure center of X axis and movement elliptical area under open and closed eyes were lower and the score of Berg balance scale was higher after treatment than those before(P<0.01).After treatment, the scores of Burke inclination scale and absolute value of mean pressure center of X axis and movement elliptical area under open and closed eyes were lower and the score of Berg balance scale was higher in group A than those in group B(P<0.05).No adverse responses occurred during treatment and one week after treatment in both groups.Conclusion Whole body vibration therapy combined with exercise therapy can effectively improve the balance ability and pusher symptoms of the patients with Pusher syndrome after stroke.
