摘 要:目的分析2011-2019年广州市登革热的流行病学特征,为进一步调整登革热防控策略提供依据。方法收集2011-2019年广州市报告登革热的病例资料,采用描述性流行病学方法,比较广州市登革热疫情的时间、空间与人群分布差异。结果 2011-2019年广州市共报告登革热病例43 802例,累计报告发病率为35.37/10万。本地发病主要集中在每年的6至12月,输入病例一年四季发生。广州市登革热病例排名前4的行政区分别为白云区(27.50%)、海珠区(13.93%)、越秀区(11.16%)和荔湾区(10.38%)。境外输入来源主要分布在新加坡、马来西亚、缅甸等东南亚国家(38.56%),境内输入来源以广东省内其它城市为主(49.78%)。本地病例中女性占比高于输入病例;发病人群主要集中在20~39岁的青壮年,年龄≥60岁的老年患者占比本地大于输入;本地病例职业以家务及待业为主,输入病例中商业服务占比高于本地。结论 2011-2013年广州市登革热病例呈上升趋势,2015年大幅下降,随后发病频率与流行强度明显增加,2019年再次出现登革热流行高峰。广州市本地登革热疫情波及范围近年来呈现市中心向周边地区扩散趋势,流行特点发生变化,需进-步调整登革热防控策略,加强广州市蚊媒监测系统,提高边境检疫部门应急处置工作,在登革热疫情初期进行有效控制。Objective To analyze the epidemiological characteristics on dengue fever cases in Guangzhou City from 2011 to 2019, so as to adjust further prevention and control strategies of dengue fever. Methods Indigenous and imported cases of dengue fever in Guangzhou City from 2011 to 2019 were collected and descriptive epidemiological methods were used to compare the temporal, spatial and population distribution of dengue fever. Results A total of 43 802 cases of dengue fever were reported in Guangzhou City from 2011 to 2019, with a total reported incidence of 35.37/100 000. The indigenous incidence mainly occurred from June to December every year, while the imported cases happened throughout the year. The top four areas of dengue cases in Guangzhou City were Baiyun District(27.50%), Haizhu District(13.93%), Yuexiu District(11.16%) and Liwan District(10.38%). Majority of the imported cases were derived from Southeast Asian countries such as Singapore, Malaysia and Myanmar(38.56%). The sources of domestic imported cases were mainly from other cities in Guangdong Province(49.78%). The proportion of female in local infected cases was higher than that of imported cases. The majority of the patients were young adults aged 20-39 years old, and the proportion of the elderly patients aged ≥60 years old was higher than that of the imported patients. The occupations of local cases were mainly domestic work and unemployment, and the proportion of commercial services in imported cases was higher than that of local cases. Conclusion The incidence of dengue fever in Guangzhou City shows an increasing trend from 2011 to 2013 and decreases significantly in 2015, following to the incidence and prevalence increases dramatically and reaches the highest peak of dengue again in 2019. In recent years, the number and scale of dengue outbreak in Guangzhou City spread from downtown to surrounding areas, and the epidemic characteristics have changed, which is necessary to adjust further prevention and control strategy.
