摘 要:目的探索系统性红斑狼疮(SLE)患者免疫球蛋白GN-糖基与血清胱抑素C(Cystatin C)水平的关系,为疾病变化的联合生物标志物的研究提供基础。方法研究对象选自2018—2019年在聊城市人民医院风湿免疫科门诊及住院的240例SLE女性患者。采用超高效液相色谱(UPLC)检测免疫球蛋白G N-糖基化水平,采用颗粒增强散射免疫比浊法(PENIA)进行血清胱抑素C(CysC)的检测。免疫球蛋白G N-糖基与血清CysC水平的相关性分析应用Spearman秩相关分析以及多元线性逐步回归模型。结果多元线性逐步回归分析显示,系统性红斑狼疮患者免疫球蛋白G N-糖基直接测量指标GP8(t=-2.460,P=0.016),GP10(t=2.283,P=0.025),GP19(t=3.570,P=0.001),GP24(t=-2.743,P=0.008)与血清CysC水平呈独立相关关系。结论系统性红斑狼疮患者中IgG N-糖基水平改变与血清CysC水平可能存在关联,为疾病变化的联合标志物的研究提供基础。Objective To explore the relationship between immunoglobulin G N-glycosylation and serum Cystatin C level in patients with systemic lupus erythematosus(SLE), and to provide a basis for the study of combined biomarkers of disease changes.Methods The study subjects were selected from 240 female patients with SLE who were hospitalized and outpatients of the Department of Rheumatology and Immunology of Liaocheng People’s Hospital from 2018 to 2019. Ultra-performance Liquid Chromatography(UPLC) was used to detect the levels of immunoglobulin G N-glycosylation, and the Particle-Enhanced NephelometryImmunoturbidimetric Assay(PENIA) was adopted to examine serum Cystatin C(CysC). The correlation between immunoglobulin G N-glycosylation and serum Cystatin C level was analyzed by theSpearman rank correlation analysis and multiple linear stepwise regression model.Results Multiple linear stepwise regression analysis showed that the direct measurement index of plasma IgG N-glycosylation in patients with systemic lupus erythematosus was GP8(t=-2.460, P=0.016), GP10(t=2.283, P=0.025), GP19(t= 3.570, P=0.001), GP24(t=-2.743, P=0.008), and the serum Cystatin C levels were independently correlated to immunoglobulin G N-glycosylation content.Conclusion Changes in IgG N-glycosylation levels in patients with systemic lupus erythematosus may be related toserum Cystatin C levels, which provides a basis for the study of combined markers of disease changes.
