摘 要:目的探讨云南地区宫颈癌(CC)和宫颈上皮内瘤样病变(CIN)患者宫颈手术组织人乳头瘤病毒(HPV)感染情况。方法选取云南地区CC患者703例、CIN患者263例,采集所有患者术后宫颈病变组织50~100 mg,采用导流杂交基因芯片技术(HybriMax)对2组患者宫颈病变组织进行HPV检测,分析HPV感染及亚型分布情况。结果CC组和CIN组患者HPV检出率均超过95%,2组比较差异无统计学意义(P>0.05);2组患者中检出率最高的HPV亚型均为HPV16,CC组和CIN组患者中检出率第二的HPV亚型分别为HPV18和HPV58,CC组患者的HPV16和HPV18的检出率明显高于CIN组(P<0.001),CIN组中HPV52、HPV56和HPV58的检出率均明显高于CC组(P<0.001);CC组患者单一感染率明显高于CIN组(P<0.001),2组患者单一感染中检出率最高的HPV型别均为HPV16、且CC组患者HPV16检出率明显高于CIN组(P<0.001),CIN患者三重及以上感染率高于CC患者(P<0.001)。结论HPV感染是诱导CC发生的主要因素,其中HPV16感染是云南地区CC患者中最常见的HPV型别,其次为HPV18。Objective To investigate the prevalence of infection by human papillomavirus(HPV)in cervical surgery tissues of patients with cervical cancer(CC)or cervical intraepithelial neoplasia(CIN)in Yunnan.Methods A total of 703 patients with CC and 263 patients with CIN in Yunnan province were selected.Cervical lesions(50-100 mg/patient)were collected from all patients after surgery.The HPV types were genotyped using flow-through hybridization HPV genotyping gene chips(HybriMax).HPV was analyzed to investigate the distribution and subtypes of infection.Results The detection rates of HPV in both the CC group and the CIN group exceeded 95%,but no statistically significant difference between two groups(P>0.05).The HPV subtype with the highest detection rate in two groups was HPV16.The second HPV subtype was HPV18 in CC group and HPV58 in CIN group.The detection rates of HPV16 and HPV18 in CC group were significantly higher than those in CIN group(P<0.001).The detection rates of HPV52,HPV56,and HPV58 in CIN group were significantly greater than those of CC group(P<0.001).The infection rate of a single HPV subtype in CC group was significantly higher than that of CIN group(P<0.001).Among infections with a single subtype,HPV16 had the highest detection rate in two groups.The detection rate of HPV16 was greater in CC group than that in CIN group(P<0.001).The infection rate of CIN patients with the triple infection or above was higher than that in CC patients(P<0.001).Conclusion HPV infection is the main factor that induces the occurrence of CC.HPV16 infection is the most common type of HPV among CC patients in Yunnan area,followed by HPV18.
