局转皮瓣联合PRP修复术后切口缘皮肤坏死

摘　　要：目的探讨局转皮瓣联合富血小板血浆(PRP)修复术后切口缘皮肤坏死的临床疗效。方法2013年10月-2015年10月,收治10例术后切口缘皮肤坏死的患者。皮肤坏死范围:3.5 cm×3.0 cm~11.0 cm×10.0 cm;术后至切口缘皮肤坏死清创时间为15~30 d,平均15 d。伤口清创后予以局部转移皮瓣+供区游离植皮+PRP修复创面。结果术后皮瓣、肌皮瓣均成活,创面一期愈合;供区植皮均成活,切口均一期愈合,无再发感染。所有病例均获随访,随访时间12~18个月,平均15个月。皮瓣与肌皮瓣感觉与血运良好;内固定物及假体无松动、移位。结论局转皮瓣联合PRP治疗术后切口缘皮肤坏死能获得较满意临床效果。Objective To investigate the clinical effect of the treatment of skin necrosis with the combination of the transfer of the skin flap and platelet rich plasma(PRP).Methods Ten cases of skin necrosis after operation were treated in our hospital from October 2013 to October.The range of skin necrosis was 3.5 cm×3.0 cm~11.0 cm×10.0 cm,and the average time from skin necrosis to debridement was 15~30 d,the average was 15 d.After debridement,local transfer flap was combined with PRP and skin graft to repair the wound.Results The flaps,muscle flaps survived and wounds healed,the donor skin grafts survived and the incision healed in one stage.No recurrence of infection.All cases were followed up for 12 to 18 months with an average of 15 months.Flap and musculocutaneous flap had good feeling and blood supply;No internal fixation and prosthesis loosening occured displacement.Conclusion The clinical effect of the combination of the transfer flap and PRP in the treatment of incision skin necrosis after the operation is satisfactory.
