游离[足母]趾腓侧皮瓣修复指腹缺损18例

摘　　要：目的探讨应用游离[足母]趾腓侧皮瓣修复指腹缺损的临床疗效。方法2016年2月-2018年11月,应用游离[足母]趾腓侧皮瓣修复指腹缺损18例,指腹缺损面积为1.5 cm×2.0 cm~3.2 cm×6.0 cm,皮瓣切取面积为1.8 cm×2.2 cm~3.6 cm×6.2 cm,3例供区直接缝合,15例供区取中厚皮植皮。随访皮瓣外观、质地、两点辨别觉,测量手指活动度,了解皮瓣供区外形、功能恢复情况及并发症。结果18例皮瓣全部成活,随访5~18个月,皮瓣血运良好,外观逼真,无臃肿,质地柔软,两点辨别觉5.0~7.0 mm,手指屈伸功能恢复良好,按照中华医学会手外科分会功能评价标准评定,优16例,良2例。结论应用游离[足母]趾腓侧皮瓣修复指腹缺损是一种理想的手术方法。Objective To study the clinical effects of fibular flap of great toe on the repairment of finger pulp defects.Methods From February 2016 to November 2018,18 cases of finger pulp defects were repaired by using the free fibular flap of great toe.The area of finger pulp defects was 1.5 cm×2.0 cm~3.2 cm×6.0 cm,and the area of skin flap was 1.8 cm×2.2 cm~3.6 cm×6.2 cm,5 skin flap donor sites were directly sutured,and 15 skin flap donor sites were repaired by medium-thick skin grafts.The follow-up content included the appearance,texture,two-point discrimination of the flap,measurement of finger activity,shape of the flap donor area,functional recovery and complications.Results All the flaps survived in 18 cases.The follow-up was 5 to 18 months.The flaps had good blood flow,realistic appearance,soft texture and were not bloated.The two-point discrimination was 5.0 to 7.0 mm.The finger flexion and extension function recovered well.According to the functional evaluation standard of Hand surgery Branch of Chinese Medical Association,the results were excellent in 16 cases and good in 2 cases.Conclusion The clinical effect of repairing the finger pulp defects with free fibular flap of great toe is an ideal method.
