摘　　要：目的探讨应用游离腓肠内侧动脉穿支皮瓣修复手足软组织缺损的临床疗效。方法2016年1月-2020年5月,对12例手足软组织缺损,采用游离腓肠内侧动脉穿支皮瓣修复。皮瓣切取面积3.0 cm×5.0 cm~5.0 cm×7.0 cm。所有病例术后均通过门诊、微信、电话等方式定期随访,随访内容为皮瓣血运情况、外形、感觉,以及供、受区外观功能情况。结果术后12例皮瓣均成活。皮瓣供区均直接缝合,供区创面及受区创面均一期愈合。所有病例术后随访6~15个月,平均10个月,皮瓣血运正常,外形不臃肿,色泽满意,质地良好,未出现皮瓣磨损、破溃现象。皮瓣感觉:1例为S 3,4例为S 2,7例为S 1。所有病例手足运动功能均未受影响。结论游离腓肠内侧动脉穿支皮瓣血管蒂走行恒定,皮瓣外形美观,质地优良,切取后供区外形、功能损伤小,是修复手足软组织缺损的良好选择。Objective To explore the clinical effect of using free medial sural artery perforator flap to repair hand and foot soft tissue defects.Methods From January 2016 to Mayr 2020,12 cases of hand and foot soft tissue defects were repaired with free medial sural artery perforator flaps.The area of the skin flap was 3.0 cm×4.0 cm~5.0 cm×7.0 cm.All cases were followed up regularly through outpatient clinics,We Chat,telephone,etc.The content of the follow-up included the blood supply,appearance,sensation of the flaps,and the appearance and function of the donor and recipient areas.Results A total of 12 skin flaps in this group survived.Twelve cases of the flap donor were directly sutured,and the wounds of the donor area and the recipient healed.All cases in this group were followed up from 6 to 15 months,an average of 10 months.The flaps had normal blood supply,no bloated appearance,satisfactory color and good texture,and no flaps were worn or ruptured.Skin flap sensation:S31 case,S24 cases,S17 cases.The function of hand and foot movement was not affected in all cases.Conclusion The free medial sural artery perforator flap has a constant vascular pedicle,and the flap is beautiful in appearance and good in texture.After the flap is cut,the appearance and function of the donor area are small.It is a good choice for repairing soft tissue defects of hands and feet.
