中西医结合治疗老年混合痔患者的临床疗效

摘　　要：目的探讨加味四黄汤内服外敷对老年混合痔患者术后康复及炎性因子的影响。方法选取2016年4月至2018年9月于本院接受手术治疗的90例老年混合痔患者作为研究对象,按照随机数表法分为对照组与观察组,每组45例。对照组给予西医常规治疗联合高锰酸钾溶液坐浴治疗,观察组给予西医常规治疗联合加味四黄汤内服外敷治疗。比较两组术后康复效果、炎性因子水平、疼痛程度、皮肤不良反应发生率及中医证候积分。结果治疗后,观察组水肿消失、渗出物消失、创面愈合时间均短于对照组(P<0.05);治疗后,两组IL-6、CRP、VAS评分均低于治疗前,VEGF高于治疗前,且观察组IL-6、CRP、VAS评分均低于对照组,VEGF高于对照组(P<0.05);观察组皮肤不良反应发生率(4.44%)略低于对照组(13.33%),但差异无统计学意义;治疗后,观察组中医证候积分低于对照组(P<0.05)。结论对行手术治疗的老年混合痔患者使用加味四黄汤内服外敷可有效加快创面愈合,改善患者炎性因子水平,降低患者疼痛程度及中医证候积分。Objective To investigate the effect of internal and external application of Jiawei Sihuang decoction on postoperative rehabilitation and inflammatory factors in elderly patients with mixed hemorrhoids.Methods From April 2016 to September 2018,90 elderly patients with mixed hemorrhoids were selected as the research subjects,they were divided into control group and observation group according to random number table method,with 45 cases in each group.The control group was given routine Western medicine treatment with potassium permanganate solution sitting bath treatment,the observation group was given routine treatment combined with Jiawei Sihuang decoction internal and external application treatment.The effects of postoperative rehabilitation,level of inflammatory factors,degree of pain,incidence of skin adverse reactions and TCM syndrome score were compared between the two groups.Results After treatment,the edema disappeared,exudate disappeared and wound healing time in the observation group were shorter than those of the control group(P<0.05);after treatment,the scores of IL-6,CRP,and VAS in the two groups were lower than before treatment,and VEGF was higher than before treatment,the scores of IL-6,CRP and VAS in the observation group were lower than those of the control group,and VEGF was higher than that of the control group(P<0.05);the incidence of skin adverse reactions in the observation group(4.44%)was slightly lower than the control group(13.33%),but the difference was not statistically significant;after treatment,the scores of TCM syndromes in the observation group was lower than that in the control group(P<0.05).Conclusion The internal and external application of Jiawei Sihuang decoction can effectively accelerate wound healing in elderly patients with mixed hemorrhoids,improve the level of inflammatory factors in patients,reduce the degree of pain and TCM syndrome score.
