顺势双反牵引复位在股骨转子间骨折手术中的应用

摘　　要：目的探究顺势双反牵引复位法应用于股骨转子间骨折手术的治疗效果。方法选取2015年5月至2018年6月本院收治的股骨转子间骨折患者60例作为研究对象,随机分为观察组和对照组,每组30例。对照组采用传统手术切口复位法,观察组采用顺势双反牵引复位法。比较两组颈干角、侧位成角、骨折前后位重叠率、骨折侧位重叠率、手术时间、术中出血量、透视次数、骨折愈合时间及术后1年Harris评分。结果观察组复位良好率(83.33%)高于对照组(33.33%),复位一般率(13.33%)低于对照组(46.67%),复位较差率(3.33%)低于对照组(20.00%),差异有统计学意义(P<0.05);观察组侧位成角、骨折前后位置重叠率、骨折侧位重叠率均高于对照组(P<0.05);观察组手术时间、骨折愈合时间均短于对照组,术中出血量、透视次数均少于对照组,术后1年Harris评分优于对照组(P<0.05)。结论顺势双反牵引复位能促进股骨转子间骨折术后复位,且能有效缩短患者手术时间、骨折愈合时间,减少术中出血量及透视次数,促进股骨功能恢复,值得临床推广应用。Objective Explore the therapeutic effect of homeopathic double reverse traction reduction method applied to femoral intertrochanteric fracture surgery.Methods 60 patients with femoral intertrochanteric fracture admitted to our hospital from May 2015 to June 2018 were selected as research subjects and they were randomly divided into observation group and control group,with 30 cases in each group.The control group used the traditional surgical incision reduction method,and the observation group used the homeopathic double reverse traction reduction method.The cervical shaft angle,lateral angulation,anteroposterior and posterior fracture overlap rate,lateral fracture overlap rate,operation time,intraoperative blood loss,number of fluoroscopy,fracture healing time and Harris score at 1 year after surgery were compared between the two groups.Results The good reduction rate in the observation group(83.33%)was higher than the control group(33.33%),the general reduction rate(13.33%)was lower than the control group(46.67%),and the poor reduction rate(3.33%)was lower than the control group(20.00%),the difference was statistically significant(P<0.05).Lateral angulation,overlapping rate before and after fracture,and overlapping rate of lateral fracture in observation group were higher than those of the control group(P<0.05).The operation time and fracture healing time of the observation group were shorter than those of the control group,the amount of blood loss and fluoroscopy times during operation were less than those of the control group,and Harris score at 1 year after surgery was better than that of the control group(P<0.05).Conclusion Homeopathic double reverse traction reduction can promote postoperative reduction of femoral intertrochanteric fracture,and can effectively shorten the operation time,fracture healing time,reduce intraoperative blood loss and fluoroscopy times,and promote the recovery of femoral function,which is worthy of clinical application.
