摘　　要：目的分析子宫托治疗盆腔器官脱垂(POP)失败的原因,提高子宫托治疗POP的成功率。方法回顾性分析2014年5月至2017年10月本院收治的119例POP患者的临床资料,按治疗效果分为成功组(n=93)与失败组(n=26),以盆底功能障碍问卷简表20(PFDI-20)及盆底功能影响问卷简表(PFIQ-7)评价疗效,试戴成功并佩戴>1个月为佩戴成功,分析佩戴失败和放弃使用的原因。结果两组年龄、BMI、阴道分娩次数、脱垂严重程度比较差异无统计学意义;阴道全长(TVL)≤6 cm、阴道口宽度≥5 cm、全子宫切除术后、阴道未用雌激素软膏患者子宫托佩戴失败率较高。结论硅胶子宫托是治疗POP的有效方法,并发症较轻微,但阴道总长度较短、阴道口较宽、全子宫切除术史易导致子宫托放置失败。Objective To analyze the causes for the failure of the silicone pessary in the treatment of pelvic organ prolapse(POP),and improve the success rate of the pessary in the treatment of POP.Methods The clinical data of 119 POP patients admitted to our hospital from May 2014 to October 2017 were retrospectively analyzed.According to the treatment effect,they were divided into success group(n=93)and failure group(n=26).Validated prolapse quality of life questionnaire 20(pelvic floor distress inventory short form 20,PFDI-20)and pelvic floor impact questionnaire short form 7(PFIQ-7)were used to evaluate the therapeutic effect.successful pessary fitting trial occurred if woman continued to use pessary>1 month after being fitted.Analyze the reasons for wearing failure and abandoning use.Results There was no significant difference in age,BMI,number of vaginal delivery,and severity of prolapse between the two groups;the total length of vagina(TVL)≤6 cm,vaginal opening width≥5 cm,after total hysterectomy,the uterus of the vagina without estrogen ointment high rate of failure to wear.Conclusion Silicone pessary is effective way to treat POP,and the complications are generally mild.However,the total length of the vagina is short,the vaginal opening is wide,and the history of total hysterectomy can easily lead to the failure of pessary placement.
