老年人院外急性气道梗阻的救治

摘　　要：目的探讨运用医疗优先调派系统(Medical Priority Dispatch System,MPDS)流程通过电话指导院外呼救者(第一目击者)实施急性气道梗阻救治对患者的预后及影响,寻求院外老年人急性气道梗阻在呼救时的救治措施。方法从济南市急救中心呼救数据库中选取2020-01-01至2020-12-31共126例老年急性气道梗阻患者,根据第一目击者是否接受调度员电话指导施救将患者分为观察组(呼救者接受调度员指导)67例和对照组(呼救者不接受调度员指导)59例。通过电子病历整理统计两组患者呼救资料、抢救是否成功、患者预后等情况。结果观察组和对照组的比较差异在有无基础性疾病、性别等无统计学意义(P>0.05)。施救时间、施救结果二者差异有统计学意义(P<0.05)。结论现场第一目击者(呼救者)在老年人院外发生急性气道梗阻时接受调度员电话指导可有效提高抢救成功率,改善患者预后,值得院外呼救推广使用。Objective To explore the prognosis and influence of applying Medical Priority Dispatch System(MPDS)to direct the rescue by telephone(with the first witness)on patients with acute airway obstruction,and to seek first-aid measures for out-of-hospital aged patients with acute airway obstruction.Methods A total of 126 elderly patients with acute airway obstruction were selected,from January 1,2020 to December 31,2020,from the first aid database of the Jinan Emergency Medical Center.According to whether the first witness received telephone instruction from the dispatcher,the patients were divided into two groups:67 cases in the observation group and 59 cases in the control group.The two groups'electronic medical records were collected and the data of calling,rescuing and prognosis were analyzed.Results There was no significant difference between the observation group and the control group in gender and the existence of underlying disease(P>0.05).There were significant differences in the rescue time and the rescue result(P<0.05).Conclusion The first-witness(rescuer)who receives the dispatcher's telephone instruction in rescuing elderly acute airway obstruction patients outside the hospital can effectively improve the success rate of rescue and the prognosis of patients.
