HFUS引导下的乳腺穿刺活检在乳腺微小病灶定性诊断中的应用价值

摘　　要：目的:探讨高频超声(HFUS)引导下的乳腺穿刺活检在乳腺微小病灶定性诊断中的价值。方法:108例乳腺微小病灶患者,均行HFUS检查及HFUS引导下的乳腺穿刺活检,以手术病理学检查结果为"金标准",比较HFUS检查及HFUS引导下的乳腺穿刺活检的效能,以及不同病灶形态及超声特征的乳腺微小病灶的恶性检出率。结果:HFUS引导下的乳腺穿刺活检乳腺微小病灶的灵敏度95.4%、特异度98.5%、准确度97.2%,均高于HFUS检查的60.5%、83.1%、74.1%,差异有统计学意义(P<0.05)。不规则形态的乳腺微小病灶的恶性检出率高,超声低回声均匀、边缘毛糙、显示血流、阻力指数>0.7的乳腺微小病灶的恶性检出率高。结论:HFUS引导下的乳腺穿刺活检应用于乳腺微小病灶的定性诊断能提高诊断效能。Objective:To discuss the value of high frequency ultrasound(HFUS)-guided breast needle biopsy in the qualitative diagnosis of minimal lesion of breast.Methods:Of the 108 patients with minute lesion of breast,they all underwent HFUS examinations and HFUS-guided breast needle biopsy.The results of surgical pathology examination were taken as the gold standard to compare the efficacy of HFUS examination and HFUS-guided breast needle biopsy,and the malignant detection rate of breast microlesions with different lesion morphology and ultrasonic characteristics under the two detection methods.Results:The sensitivity,specificity and accuracy of HFUS-guided breast needle biopsy were 95.4%,98.5%and 97.2%,respectively,which were all higher than the respective results of 60.5%,83.1%and 74.1%by HFUS examination,and the differences were of statistical significances(P<0.05).The malignant detection rate of minimal breast lesions with irregular shape was higher,and the ultrasonography was homogeneous with low echo and rough edge,showing that the detection rate of malignancy was higher in minimal breast lesions with the resistance index of blood flow larger than 0.7.Conclusion:The application of HFUS-guided breast needle biopsy can improve the diagnostic efficiency in the qualitative diagnosis of patient with small breast lesions.
