功能性电刺激结合综合康复疗法对脑卒中后肩关节半脱位患者的治疗作用

摘　　要：目的:研究功能性电刺激结合综合康复疗法对脑卒中后肩关节半脱位患者肩关节活动度及上肢运动功能的治疗作用。方法:肩关节半脱位患者75例,随机分为两组。对照组38例,采用综合康复疗法治疗;观察组37例,在对照组基础上,进行功能性电刺激治疗。治疗20 d后,观察、分析两组治疗的有效率,肩关节活动度、FMA-UE评分及VAS评分。结果:观察组总有效94.6%,高于对照组76.3%,差异有统计学意义(P<0.05);治疗后,两组肩关节活动度及FMA-UE评分高于治疗前,且观察组高于对照组;两组VAS评分在治疗后降低,且观察组低于对照组(均P<0.05)。结论:功能性电刺激联合综合康复疗法提高脑卒中后肩关节半脱位患者的治疗效果,增强肩关节活动度及上肢运动功能。Objective:To investigate the therapeutic effects of functional electrical stimulation combined with comprehensive rehabilitation therapy on motionrange of shoulder joint and upper limb function in patients with shoulder subluxation after stroke.Methods:75 patients with shoulder subluxation after stroke were randomized into 2 groups.37 patients in the control group received comprehensive rehabilitation therapy,while another 38 patients in the observation group were treated with functional electrical stimulation on the basis of the control group.After 20 d treatment,the clinical efficacy,range of motion,FMA-UE score and VAS score of patients were compared between the two groups.Results:The total effective rate of observation group was 94.6%,which was higher than 76.3%of control group,and the difference was statistically significant(P<0.05).After treatment,the shoulder joint motion and FMA-UE score in both groups was higher than that before treatment,and the observation group’s shoulder joint motion and FMA-UE score was higher than that of the control group;VAS score in both groups decreased after treatment,and the observation group’s VAS was lower than that of control group(P<0.05).Conclusion:Functional electrical stimulation combined with comprehensive rehabilitation therapy can improve the therapeutic effect of patients with shoulder subluxation after stroke and enhance the range of motion of shoulder joint and upper limb motor function.
