百令胶囊联合左卡尼汀对维持性腹膜透析患者肾功能、微炎症状态、营养状况及生活质量的影响分析

摘　　要：目的探讨百令胶囊联合左卡尼汀对维持性腹膜透析患者肾功能、微炎症状态、营养状况及生活质量的影响。方法选取2019年3月—2020年5月我院收治的行维持性腹膜透析的慢性肾衰竭86例,据治疗方法不同分为观察组46例和对照组40例。对照组在常规及维持性腹膜透析基础上加用左卡尼汀,观察组在对照组基础上加用百令胶囊;均治疗2个月。比较2组治疗前后肾功能指标[尿素(BUN)、血肌酐(SCr)],炎性因子[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]水平,营养因子[前蛋白(PA)、白蛋白(Alb)、转铁蛋白(TRF)、血红蛋白(Hb)]水平,生活质量评价量表(SF-36)评分及不良反应。结果与治疗前比较,2组治疗后BUN、SCr、CRP、TNF-α及IL-6均降低,且观察组低于对照组,差异有统计学意义(P<0.05,P<0.01)。与治疗前比较,2组治疗后PA、Alb、TRF、Hb及SF-36评分均升高,且观察组高于对照组,差异有统计学意义(P<0.01)。2组治疗期间不良反应总发生率比较差异无统计学意义(P>0.05)。结论百令胶囊联合左卡尼汀治疗维持性腹膜透析慢性肾衰竭,更有利于改善患者肾功能、微炎症状态、营养状况及生活质量,且安全性良好。Objective To investigate effects of Bailing Capsule combined with Levocarnitine on renal function,microinflammatory state,nutritional status and quality of life in patients undergoing maintenance peritoneal dialysis.Methods A total of 86 patients undergoing maintenance peritoneal dialysis admitted during March 2019 and May 2020 were selected and divided into observation group(n=46)and control group(n=40)according to different therapeutic methods.Control group was given Levocarnitine on the basis of conventional treatment and maintenance peritoneal dialysis,while observation group was supplemented with Bailing Capsule on the basis of treatment for control group.Both groups were treated for 2 months.Levels of renal function indexes[blood urea nitrogen(BUN),serum creatinine(SCr)],levels of inflammatory factor[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)],levels of nutritional factors[proalbumin(PA),albumin(Alb),transferrin(TRF),hemoglobin(Hb)],Short Form 36 Questionnaire(SF-36)score and incidence rate of adverse reactions before and after treatment were compared between two groups.Results Compared with those before treatment,BUN,SCr,CRP,TNF-αand IL-6 levels were significantly decreased after treatment in two groups,and the levels in observation group were significantly lower than those in control group(P<0.05,P<0.01);PA,Alb,TRF and Hb levels and SF-36 scores were significantly increased in two groups,and the levels and score in observation group were significantly higher than those in control group(P<0.01).There was no significant difference in the total incidence rate of adverse reactions during the treatment between two groups(P>0.05).Conclusion Bailing Capsule combined with Levocarnitine may improve renal function,microinflammatory state,nutritional status and quality of life with good safety in treatment of patients undergoing maintenance peritoneal dialysis.
