摘 要:目的总结贝那普利在慢性肾小球肾炎患者体内的药动力学特征。方法选取2018年3月-2019年7月厦门市海沧医院收治的慢性肾小球肾炎患者37例,患者均口服贝那普利20 mg后,通过高效液相色谱仪—串联质谱法测量患者体内的贝那普利及其代谢物浓度变化,最终计算出贝那普利的药动力学参数。结果研究发现,慢性肾小球肾炎患者的贝那普利主要药动力学参数的血浆浓度—时间曲线下面积为(78.36±27.63)μg·h^(-1)·L^(-1),最大血药浓度参数为(76.93±2.03)μg/L,其中血药达到峰值的时间为(0.76±0.08) h。而从贝那普利的代谢物来看,贝那普利拉的浓度—时间曲线下面积为(342.15±85.71)μg·h^(-1)·L^(-1),最大血药浓度为(50.36±7.16)μg/L,血药浓度到达峰值的时间为(1.79±0.13) h。结论贝那普利在慢性肾小球肾炎患者体内的药动力学水平良好,最大血药浓度、血药浓度达到峰值时间满意,证明该药物可以成为临床治疗慢性肾小球肾炎的首选,值得推广应用。Objective Summarize the pharmacokinetic characteristics of benazepril in patients with chronic glomerulonephritis. Methods Select 37 patients with chronic glomerulonephritis admitted to Xiamen Haicang Hospital from March2018 to July 2019. After oral administration of 20 mg benazepril,the patients were measured by high performance liquid chromatography-tandem mass spectrometry. The concentration of benazepril and its metabolites changes,and finally the pharmacokinetic parameters of benazepril are calculated. Results The study found that the area under the plasma concentration-time curve of the main pharmacokinetic parameters of benazepril in patients with chronic glomerulonephritis was( 78. 36 ± 27. 63)μg·h^(-1)·L^(-1). The maximum blood drug concentration parameter is( 76. 93 ± 2. 03) μg/L,and the time for the blood drug to reach the peak value is( 0. 76 ± 0. 08) h. From the perspective of the metabolites of benazepril,the area under the concentration-time curve of benazeprilat is( 342. 15 ± 85. 71) μg·h^(-1)·L^(-1),and the maximum blood concentration is( 50. 36 ± 7. 16)μg/L,the time to reach the peak blood concentration is( 1. 79 ± 0. 13) h. Conclusion The pharmacokinetic level of benazepril in patients with chronic glomerulonephritis is good,and the maximum blood concentration and the time to reach the peak blood concentration are satisfactory. Benazepril can become the first choice for clinical treatment of chronic glomerulonephritis,and it is worthy of promotion and application.
