205例HIV阴性梅毒患者脑脊液白蛋白商数及IgG指数分析

摘　　要：目的:了解HIV阴性梅毒患者中脑脊液白蛋白商数及IgG指数的分布特征。方法:共收集205例梅毒患者脑脊液标本,其中124例非神经梅毒,81例神经梅毒。使用SPSS 25.0软件进行统计学分析,计算两组患者白蛋白商数及IgG指数的敏感性、特异性及诊断效率。结果:神经梅毒组白蛋白商数及IgG指数分别为(6.98±3.80)和(1.51±1.16)高于非神经梅毒组(4.63±2.03)和(0.67±0.48),差异有统计学意义(均P<0.001)。在神经梅毒患者中,脑脊液TRUST阳性患者IgG指数均值为(2.14±1.31)较阴性患者(1.10±0.84)高,差异有统计学意义(P<0.001)。白蛋白商数在神经梅毒诊断方面,敏感性为73.33%,特异性66.29%,阳性预测值27.16%,阴性预测值93.55%,诊断效率67.32%。IgG指数在神经梅毒诊断方面,敏感性为59.60%,特异性79.25%,阳性预测值72.84%,阴性预测值67.74%,诊断效率69.76%。结论:白蛋白商数及IgG指数可用于神经梅毒的辅助诊断。Objective:To analyze the albumin quotient(QAlb)and IgG index in HIV-negative syphilis patients.Methods:205 syphilis inpatients were collected including 124 non-neurosyphilis patients and 81 neurosyphilis patients.The sensitivity,specificity and diagnostic efficiency of QAlb and IgG index was calculated use SPSS 25.0 software.Results:The QAlb and IgG index were(6.98±3.80)and(1.51±1.16)in the neurosyphilis patients which were higher than that in the non-neurosyphilis patients(4.63±2.03 and 0.67±0.48),with significant differences(P<0.001),the IgG index of the neurosyphilis patients was(2.14±1.31)in TRUST positive group,which was higher than that in negative group(1.10±0.84),with a significant difference(P<0.001).The sensitivity of QAlb and IgG index in the diagnosis of neurosyphilis was 73.33%and 59.60%,specificity was 66.29%and 79.25%,positive predictive value was 27.16%and 72.84%,negative predictive value was 93.55%and 67.74%,and the diagnostic efficiency was 67.32%and 69.76%,respectively.Conclusion:The QAlb and IgG index can be used for the auxiliary diagnosis of neurosyphilis.
