高危型HPV E6/E7mRNA在ASCUS患者临床分流中的应用研究

摘　　要：目的评价高危型人乳头状瘤病毒(human papilloma virus,HPV)E6/E7mRNA检测方法在无明确诊断意义的不典型鳞状细胞(atypical squamous cells undefined significance,ASCUS)患者临床分流中的应用价值。方法选取2017年1月—2019年6月就诊于连云港市妇幼保健院妇科门诊宫颈细胞学检查结果为ASCUS,接受高危型HPV DNA和HPV E6/E7mRNA检查,进行阴道镜检查有活检病理检查结果的338名患者。分析HPV E6/E7mRNA与阴道镜活检病理结果的相关性。结果高危型HPV E6/E7mRNA在正常组、LSIL组、≥HSIL组的阳性检出率逐步升高(19.27%、68.72%、92.75%),差异有统计学意义。ROC曲线显示,高危型HPV E6/E7mRNA预测值更优;233例HPV DNA阳性患者的阴道镜病理结果≤LSIL组的HPVE6/E7mRNA阳性率为36.3%,与≥HSIL组的HPV E6/E7mRNA阳性率(95.4%)比较,差异有统计学意义。结论高危型HPV E6/E7mRNA检测可作为ASCUC患者分流管理的有效措施。Objective To evaluate the application of high-risk human papilloma virus(HPV)E6/E7 mRNA detection method for distribution management of patients with atypical squamous cells undefined significance(ASCUS).Methods A total of 338 patients who were admitted in Lianyungang Maternal and Child Health Hospital from January 2017 to June 2019 for cervical cytological examination and diagnosed with ASCUS were selected.They underwent high-risk HPV DNA genotyping,high-risk HPV E6/E7 mRNA examination and colposcopy biopsy.The correlation between HPV E6/E7mRNA and the pathological results of colposcopy was analyzed.Results The positive rate of high risk HPV E6/E7mRNA gradually increased in the normal group,the LSIL group and the HSIL group(19.27%,68.72%,and 92.75%respectively),with statistical difference.According to ROC curve,the predictive value of high-risk HPV E6/E7mRNA was better.The positive rate of HPVE6/E7mRNA was 36.3%in 233 cases with positive HPV DNA,which was statistically different from the positive rate of HPV E6/E7mRNA in the HSIL group(95.4%).Conclusions Detection of high-risk HPV E6/E7mRNA is useful for distribution management of ASCUC patients.
