外痔静脉丛剥离联合PPH治疗重度混合痔36例疗效观察

摘　　要：为探讨外痔静脉丛剥离联合PPH治疗重度混合痔的临床效果,将36例重度混合痔患者作为观察对象,均采用外痔静脉丛剥离联合PPH治疗。观察其临床疗效,手术时间及并发症情况。结果显示,本组36例全部治愈,治愈率为100%。手术用时40~50min,平均45min;平均住院时间为8d。2例击发吻合器时出现下腹牵引痛,击发后疼痛减轻;1例出现吻合口血肿,局部压迫后血肿消除。术后患者疼痛轻微,无需口服止痛药物。2例有肛门坠胀不适感,1周后症状消失。随访6~12个月,36例无1例复发,未见肛门畸形、排便障碍、肛门狭窄及出血等。结果表明,外痔静脉丛剥离联合PPH治疗重度混合痔手术时间短,疼痛小,出血少,恢复快,疗效满意。This study was to investigate clinical effect of PPH combined with dissection for venous plexus of external hemorrhoids in the treatment of severe mixed hemorrhoids,enrolled 36 cases of severe mixed hemorrhoids as research objects:all received above combination procedure,then,their clinical effect,operating time and complications were observed.As results,all patients were cured(i.e,curative rate was up to100%);operating time lasted 40-50 min(averaged 45 min);averaged hospitalization time was 8 days;2 cases suffered hypogastric tugging(pain)when trigger stapler,after trigger,pain relieved;1 case suffered stoma hematoma,through locally pressing,hematoma disappeared;after surgery patients felt slight pain,no need to orally administer drugs to stop pain;2 cases felt anus’bearing-down&swelling,1 week later the feeling disappeared;the follow-up of 6-12 months did not find any recurrence cases,also anal deformity,defecatory disorder,as well as anal stenosis and bleeding,etc.Resutls show that PPH combined with dissection for venous plexus of external hemorrhoids in the treatment of severe mixed hemorrhoids has such merits as short operating time,slight pain,less bleeding,fast recovery,as well as satisfactory effect.
