摘　　要：目的探讨医院-社区-家庭联合护理模式对社区解锁精神病患者症状缓解及再住院率的影响。方法选取2018年7月—2020年7月社区解锁精神病患者的108例作为研究对象,按照性别、年龄、病程匹配的原则分为对照组和观察组,各54例。对照组采用常规护理方案,观察组采取医院-社区-家庭联合护理模式。分别采用简明精神病评定量表(BPRS)、社会功能缺陷筛选量表(SDSS)比较两组的精神病性症状严重程度及社会功能恢复情况,观察两组再住院率。结果两组患者BPRS、SDSS评分均随时间延长而降低,护理干预6个月、12个月末时,BPRS、SDSS评分均有下降,且观察组下降幅度优于对照组,组间比较差异具有统计学意义(P<0.05)。护理干预结束后观察组再住院率低于对照组,组间比较差异具有统计学意义(P<0.05)。结论医院-社区-家庭联合护理模式能够减轻社区解锁患者精神病性症状,改善患者恢复,降低再住院率。Objective To explore the effect of hospital-community-family joint nursing model on the symptom relief and re-hospitalization rate of psychiatric patients.Methods To select 108 cases of mental illness patients who were unlocked in the community from July 2018 to July 2020 as the study subjects,and according to the principle of matching gender,age,and disease course,the study subjects were divided into a control group and an observation group,with 54 cases in each group.The control group adopted the conventional nursing plan,and the observation group adopted the hospital-community-family joint nursing model.Brief psychiatric rating scale(BPRS)and social disability screening schedule(SDSS)were used to compare the severity of psychotic symptoms and the recovery of social function between the two groups,and the re-hospitalization rate of the two groups were observed.Results The BPRS and SDSS scores of the two groups of patients decreased with time.At the end of 6 months and 12 months of nursing intervention,the BPRS and SDSS scores decreased,and the decrease rate of the observation group were better than those of the control group,and the differences between the groups were statistically significant(P<0.05).The re-hospitalization rate of the observation group was lower than that of the control group after the nursing intervention,and the differences between the groups were statistically significant(P<0.05).Conclusion The hospital-community-family joint nursing model can alleviate the psychotic symptoms of unlocked patients in the community,improve the recovery of patients,and reduce the rate of re-hospitalization.
