经会阴实时三维超声在女性尿道周围囊性病变中的诊断价值

摘　　要：目的探讨经会阴实时三维超声在女性尿道周围囊性病变中的诊断价值。方法回顾分析我院超声数据库中诊断为尿道周围囊性病变的女性患者经会阴实时三维超声声像图,评估每个病灶的位置、数目、大小、分隔、内部回声、血流、Valsalva动作时病灶相对尿道及阴道的移动情况及TUI显示与尿道连接情况特征。以手术或临床诊断为诊断标准。结果与PC、VC相比,UD多见钙化(P<0.05),TUI可见尿道连接(P<0.000),与UD、PC相比,VC可见与尿道的相对运动(P<0.001),少见囊内分隔(P<0.05),血流信号少/缺乏(P<0.05),而PC与UD常出现与阴道的相对运动。结论经会阴实时三维盆底超声可鉴别UD、PC与VC,为临床干预及手术处理提供重要的影像学信息。Objective To explore translabial real-time 3 D ultrasound imaging findings in the diagnosis of female peri-urethral cystic lesions. Methods We searched our ultrasonic database retrospectively for translabial real-time 3 D ultrasonographic findings of female patients with periurethral cystic lesions.Each patient was evaluated location, the number of lesion, size, interval, internal echo, blood flow, relative movement between lesions and urethral and vagina on valsalva maneuver, and the connection between urethra and lesions shown by tomographic ultrasound imaging(TUI).The surgery or clinical confirmation were the diagnostic standards. Results Compared with PC and VC,UD was common calcification(P<0.05),TUI could show the gap between urethra and diverticulum(P<0.000).Compared with UD and PC,VC showed relative movement with urethra(P<0.000),and less intracapsular septum(P<0.05),less or lack of blood flow signal(P<0.05).UD and PC more often showed relative movement with vagina. Conclusions Translabial real-time 3 D ultrasound can differential diagnosis of UD,PC and VC,and provides important imaging information for clinical intervention and surgical management.
