评价腰-硬联合麻醉在糖尿病下肢血管病变手术中的应用

摘　　要：目的探究在糖尿病下肢血管病变(lower extremity arterial disease indiabetic patient,LEADDP)手术中应用腰-硬联合麻醉的价值。方法筛选该院2019年1-12月收治的64例LEADDP手术患者,随机分为观察组(34例,采用腰-硬联合麻醉手术)和对照组(30例,采用全身麻醉手术),比较两组患者的麻醉效果、VAS评分、不良反应、麻醉满意度和血压水平。结果观察组的麻醉起效时间、手术时间和术后复苏时间均显著优于对照组,差异有统计学意义(P<0.05);观察组在术后2 h、12 h、24 h的VAS评分显著低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率(5.88%)显著低于对照组(26.67%),差异有统计学意义(P<0.05);观察组的麻醉满意度(97.06%)显著高于对照组,差异有统计学意义(P<0.05);术前,两组患者的血糖水平差异无统计学意义(P>0.05),术后,观察组的血糖水平显著优于对照组,差异有统计学意义(P<0.05);两组均未发生低血糖现象。结论LEADDP患者的手术治疗中,采取腰-硬联合麻醉方式,具有良好的麻醉效果,可明显缩短术中麻醉生效时间和苏醒时间,继而缩短手术整体时间,缓解患者手术后的疼痛情况,降低不良反应发生率,提高麻醉满意度,合理控制血糖水平,值得参考。Objective To explore the value of combined lumbar-epidural anesthesia in lower extremity arterial disease indiabetic patient(LEADDP)surgery.Methods A total of 64 patients undergoing LEADDP surgery in the hospital from January to December 2019 were selected and randomly divided into observation group(34 cases,combined spinal-epidural anesthesia surgery)and control group(30 cases,general anesthesia surgery),compared the anesthesia effect,VAS score,adverse reactions,anesthesia satisfaction and blood pressure levels of the two groups of patients.Results The observation group's anesthesia onset time,operation time and postoperative resuscitation time were significantly better than those of the control group,the difference was statistically significant(P<0.05);the observation group had significantly lower VAS scores at 2 h,12 h,and 24 h after surgery in the control group,the difference was statistically significant(P<0.05);the incidence of adverse reactions in the observation group(5.88%)was significantly lower than the control group(26.67%),and the difference was statistically significant(P<0.05);the anesthesia satisfaction of the observation group(97.06%)was significantly higher than that of the control group,and the difference was statistically significant(P<0.05);before the operation,there was no statistically significant difference in blood glucose levels between the two groups(P>0.05),and the blood glucose level of the observation group after the operation was significantly better than that of the control group,and the difference was statistically significant(P<0.05);there was no hypoglycemia in the two groups.Conclusion In the surgical treatment of LEADDP patients,combined spinal-epidural anesthesia has a good anesthesia effect,which can significantly shorten the effective time and recovery time of intraoperative anesthesia,and then shorten the overall operation time,relieve the patient’s postoperative pain,and reduce adverse incidence of reaction,the improvement of anesthesia satisfaction,and the reasonable c
