Acapella Duet联合雾化吸入N-乙酰半胱氨酸对稳定期支气管扩张症患者的疗效观察

摘　　要：目的观察呼吸振动正压排痰装置Acapella Duet联合雾化吸入N-乙酰半胱氨酸,对稳定期支气管扩张症患者肺功能、圣乔治呼吸问卷评分(SGRQ评分)、支气管扩张症严重程度评分(FACED评分)的影响,探讨其临床价值。方法分析2018年1月至2019年6月我院呼吸科出院的支气管扩张症稳定期患者86例,分为观察A组、观察B组和对照组。对照组患者出院后对症治疗,观察B组在此基础上加用Acapella Duet振动正压排痰,观察A组Acapella Duet连接雾化吸入N-乙酰半胱氨酸,总疗程6个月。比较3组患者治疗前后肺功能、SGRQ评分、FACED评分的变化,以及3组患者之间的差异。结果观察A组、观察B组治疗6月后肺功能指标与治疗前及对照组比较,均有明显改善(P<0.05);SGRQ评分、FACED评分与治疗前及对照组比较,均有明显下降(P<0.05);且观察A组较观察B组差异更明显(P<0.05)。结论Acapella Duet联合雾化吸入N-乙酰半胱氨酸能显著改善稳定期支气管扩张症患者的肺功能,提高生活质量及减轻严重程度,从而改善预后。Objective To observe the effect of Acapella Duet combined with N-acetylcysteine(NAC)inhalation on lung function test,the score of SGRQ(modified St.George’s Respiratory Questionnaire)and the score of FACED(including FEV1%,age,colonization by Pseudomonas aeruginosa,radiological extension,dyspnoea)of bronchiectasis patients at stable stage,and therefore to explore its clinical value.Methods 86 patients diagnosed with bronchiectasis at stable stage were enrolled from Respiratory Department of Shanghai Punan Hospital from January 2018 to June 2019.All selected patients were randomly divided into the observation group A,the observation group B and the control group.The control group was given routine treatment.On the basis,the group B received oscillatory positive expiratory pressure devices Acapella Duet and the group A was observed with N-acetylcysteine inhalation connected with a special interface besides Acapella Duet.The total course of treatment lasted for 6 months.The outcome of lung function test,the score of SGRQ and FACED were compared in each group before and 6 months after treatment.Results 6 months after treatment,the indexes of lung function improved more significantly in the group A and the group B than before treatment and in the control group(P<0.05).The scores of SGRQ and FACED were significantly lower in the group A and the group B than those before treatment and in the control group(P<0.05).The indexes in the group A were significantly different from those in the group B(P<0.05).Conclusion Acapella Duet combined with NAC inhalation can significantly improve pulmonary function and survival quality of patients with bronchiectasis at stable stage,and reduce the severity of bronchiectasis,thus making the prognosis better.
