摘  要:目的研究慢阻肺患者炎症因子与支气管黏膜组织自噬作用之间的相关性。方法选取上海交通大学医学院附属新华医院呼吸科20例慢阻肺患者和10例健康者,分为3组:A组(轻度+中度组)、B组(重度+极重度组)和对照组。采集血、肺泡灌洗液(BALF)、支气管黏膜组织。检测血清、BALF中炎症因子水平及支气管黏膜组织自噬作用。结果(1)与对照组相比,血清IL-6、IL-8及TNF-α在A组无统计学意义,B组明显增高(IL-6P<0.05,IL-8、TNF-αP<0.01)。BALF中A组和B组IL-6水平均明显升高(P<0.05),B组IL-8及TNF-α水平明显增高(TNF-αP<0.05,IL-8P<0.001);(2)与对照组相比,A组、B组中Atg5的mRNA及Beclin1的蛋白表达在均显著低于对照组(P<0.01),LC3B的蛋白表达均显著降低(A组P<0.05,B组P<0.01),而P62的mRNA及蛋白表达均显著增加(P<0.01);(3)血清、BALF中IL-8水平与支气管黏膜自噬作用呈负相关(r<0,P<0.01)。结论IL-6、IL-8、TNF-α在慢阻肺的进展过程中明显升高。自噬作用在慢阻肺的进展过程中明显降低。IL-8水平的升高可能与慢阻肺患者自噬受损有关。Objective To explore the correlation between inflammatory factors and autophagy of bronchial mucosa in patients with chronic obstructive pulmonary disease(COPD).Methods It selected 10 cases of healthy subjects and 20 patients with COPD from the Department of Respiratory Medicine of Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine,and they were divided into the control group,the group A(GOLD1+GOLD2),and the group B(GOLD3+GOLD4)according to the severity of disease.Their serum,bronchoalveolar lavage fluid and bronchial mucosa were collected.The levels of inflammatory factors were detected in serum and BALF.The expressions of autophagy related genes and proteins were detected in bronchial mucosa.Results(1)Compared with the control group,the levels of IL-6,IL-8 and TNF-αin serum showe not statistical significance in the group A,but they were significantly higher in the group B(IL-6:P<0.05;IL-8/TNF-α:P<0.01).In BALF,the level of IL-6 in the groups A and B increased significantly(P<0.05),and the levels of IL-8 and TNF-αin the group B increased significantly(TNF-α:P<0.05;IL-8:P<0.001).(2)Compared with the control group,the expression of Atg5 mRNA and Beclin1 protein in the groups A and B was significantly lower than that in the control group(P<0.01),and the protein expression of LC3B was significantly reduced(P<0.05 in the group A,P<0.01 in the group B),while the mRNA and protein expression of P62 increased significantly(P<0.01).(3)The level of IL-8 in serum and BALF were negatively correlated with bronchial mucosal autophagy(r<0,P<0.01).Conclusion IL-6,IL-8,and TNF-αincrease significantly during the progression of COPD.Autophagy is significantly reduced during the progression of COPD.The increase of IL-8 level may be related to the impaired autophagy in COPD patients.
