摘　　要：目的探讨和血明目片联合视网膜激光光凝对视网膜静脉阻塞黄斑水肿患者的临床疗效。方法 80例患者随机分为对照组和观察组,每组40例,对照组给予视网膜激光光凝,观察组在对照组基础上加用和血明目片。检测临床疗效、视力、RI、PI、黄斑区微血管结构、SFCT、炎症因子(ICAM-1、MCP-1、IL-8、TNF-α、IL-6)、VEGF、miR-146a、sCD200、不良反应发生率变化。结果观察组临床疗效优于对照组(P<0.05)。治疗后,观察组视力、视网膜深层毛细血管层血流密度、miR-146a高于对照组(P<0.05),PI、RI、SFCT、炎症因子、VEGF、sCD200更低(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论和血明目片联合视网膜激光光凝能安全有效地改善视网膜静脉阻塞黄斑水肿患者视力、黄斑区微血管结构、SFCT,下调血清MCP-1、IL-8水平。AIM To explore the clinical effects of Hexue Mingmu Tablets combined with retinal laser photocoagulation on patients with macular edema due to retinal vein occlusion.METHODS Eighty patients were randomly assigned into control group(40 cases)for intervention of retinal laser photocoagulation,and observation group(40 cases)for intervention of both Hexue Mingmu Tablets and retinal laser photocoagulation.The changes in clinical efficacy,visual acuity,RI,PI,microvascular structure in macular area,SFCT,inflammatory factors(ICAM-1,MCP-1,IL-8,TNF-α,IL-6),VEGF,miR-146 a,sCD200 and incidence of adverse reactions were detected.RESULTS The observation group demonstrated better clinical efficacy than the control group(P<0.05).After the treatment,the observation group displayed higher visual acuity,blood flow density of deep capillary layer of retina and miR-146 a than the control group(P<0.05),along with lower PI,RI,SFCT,inflammatory factors,VEGF and sCD200(P<0.05).No obvious difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with macular edema due to retinal vein occlusion,Hexue Mingmu Tablets combined with retinal laser photocoagulation can safely and effectively improve visual acuity,microvascular structure in macular area and SFCT,and down-regulate serum MCP-1 and IL-8 levels.
