摘  要:目的检测健康孕妇不同妊娠期的血清同型半胱氨酸(Hcy)水平,为不同妊娠期的健康孕妇Hcy参考区间的确立提供依据。方法选取2017年3月至2018年3月在本院规律产检并足月分娩的健康孕妇1123例,其中妊娠早期组174例,妊娠中期组500例,妊娠晚期组449例,同时选取健康育龄妇女87例作为健康对照组。利用循环酶法检测其血清Hcy水平,并进行统计学分析。结果健康对照组Hcy水平为(8.52±3.92)μmol/L,95%参考区间为4.60~12.44μmol/L;妊娠早期组Hcy水平为(6.31±2.84)μmol/L,95%参考区间为3.47~9.15μmol/L;妊娠中期组Hcy水平为(6.01±2.96)μmol/L,95%参考区间为3.05~8.97μmol/L;妊娠晚期组Hcy水平为(7.76±3.65)μmol/L,95%参考区间为4.11~11.41μmol/L。方差分析显示,不同孕期组的Hcy水平差异有统计学意义(F=104.782,P<0.001)。多重比较Bonferroni检验显示,妊娠早期组与妊娠中期组Hcy水平比较,差异无统计学意义(P>0.05);妊娠晚期组与妊娠早期组、妊娠中期组Hcy水平比较,差异均有统计学意义(均P<0.05);且妊娠早期、中期、晚期与健康对照组比较,差异均有统计学意义(均P<0.05)。线性相关分析显示,妊娠妇女Hcy水平与其年龄、孕周在P=0.01水平(双侧)上显著相关(r=0.090、0.046)。结论健康孕妇不同妊娠期Hcy水平有差异,不同妊娠期确立不同的Hcy参考区间对于诊断妊娠期高同型半胱氨酸血症具有重要的意义。Objective To investigate the serum homocysteine concentrations in healthy pregnant women during different gestation periods and to establish the reference ranges.Methods A total of 1123 healthy pregnant women who underwent regular prenatal care and got full term in labor in our hospital from March 2017 to March 2018 were enrolled,including 174 cases in the first trimester,500 cases in the second trimester,and 449 cases in the third trimester,and 87 healthy women without pregnancy as controls.The serum homocysteine concentration was detected by enzymatic cycling assay and was statistically analyzed.Results The serum Hcy concentration in the control group was(8.52±3.92)μmol/L(95%reference range 4.60-12.44μmol/L),(6.31±2.84)μmol/L(95%reference range 3.47-9.15μmol/L)in the first trimester group,(6.01±2.96)μmol/L(95%reference range 3.05-8.97μmol/L)in the second trimester group,and(7.76±3.65)μmol/L(95%reference range 4.11-11.41μmol/L)in the third trimester group.The variance analysis showed that the difference of Hcy concentrations in different gestation periods was statistically significant(F=104.782,P<0.001).Multiple comparison Bonferroni test showed that there was no statistically significant difference in the Hcy concentration between the first trimester group and the second trimester group(P>0.05),there were statistically significant differences in the Hcy concentration between the third trimester group and the first trimester group and between the third trimester group and the second trimester group(both P<0.05),and there were statistically significant differences in the Hcy concentration between the first trimester group,the second trimester group,the third trimester group,and the control group(all P<0.05).Linear correlation analysis showed that there were positive correlations between the serum Hcy concentration of pregnant women and their age and gestational age at P=0.01 level(bilateral)(r=0.090,0.046).Conclusion There was statistcal significance in the serum Hcy concentration in healthy pregnant
