摘 要:目的:探讨对急性有机磷中毒(Acute organophosphorus pesticide poisoning,AOPP)合并酒精戒断综合症(Alcohol withdrawal syndrome,AWS)患者,急诊救治中予以右美托咪定辅助镇静的价值。方法:选取30例AOPP合并AWS患者为研究对象,按照镇静方案的不同分成咪达唑仑组与右美托咪定组,各15例,对患者均予以急诊抢救并予以气管插管镇静,两组使用不同的镇静方式,对两组干预效果进行评价。结果:在到达满意镇静深度及停药后完全清醒时间上右美托咪定组均短于咪达唑仑组,差异有统计学意义(P<0.05);镇静前后两组各指标均有一定程度的降低,其中咪达唑仑组HR、MAP、R差异有统计学意义(P<0.05),SpO2差异无统计学意义(P>0.05),而右美托咪定组HR、MAP、R、SpO2差异均无统计学意义(P>0.05)。两组镇静后各指标比较差异均有统计学意义(P<0.05)。结论:针对AOPP合并AWS患者,急诊抢救患者中使用右美托咪定辅助镇静的效果满意,对呼吸及血流动力的影响小。Objective:To explore the value of dexmedetomidine as an aid for sedation in patients with acute organophosphorus poisoning(AOPP)combined with alcohol withdrawal syndrome(AWS)in emergency treatment.Methods:Thirty patients with AOPP combined with AWS were selected as study subjects,and were divided into group A(midazolam,15 cases)and group B(dexmedetomidine,15 cases)according to different sedation schemes.They were rescued and sedated by tracheal intubation.The two groups used different sedation methods to evaluate the intervention effects of the two groups.Results:The indexes of the dexmedetomidine group were shorter than the midazolam group in reaching the satisfactory sedation depth and the time of full wakeness after withdrawal,and the differences were statistically significant(P<0.05).Calm before and after the two groups of indicators are all in a certain degree of lower and midazolam group HR,MAP,R differences were statistically significant(P<0.05).There was no statistically significantdifferences SpO2(P>0.05),and the right supporting the dexmedetomidine group HR,MAP,R,SpO2differences has no statistical significance(P>0.05).After sedation,there were statiatically significant differences in each index between the two gtoups(P<0.05).Conclusion:For patients with AOPP combined with AWS,the use of dexmedetomidine-assisted sedation in emergency rescue is satisfactory,and it has little influence on breathing and blood flow,and it is worthy of promotion.
