那屈肝素钙引起多例转氨酶升高原因分析及干预

摘　　要：目的查找引起本院妇科恶性肿瘤患者手术后转氨酶升高的药物,分析其不良反应特点,提出药学干预对策,从而降低此不良反应的发生率。方法临床药师对本院妇科2018年9月至12月期间肿瘤手术后转氨酶升高的24例患者进行不良反应与药物的关联性以及不良反应的特点分析,并给临床医师提出预防和治疗的建议。结果24例患者年龄为(49.90±7.76)岁,体质量为(56.20±7.11)kg。除1例患者考虑由化疗药物引起,其余23例转氨酶升高考虑由那屈肝素钙所致。采取调整那屈肝素钙剂量、调整监测肝功能时间、转氨酶升高后及时给予护肝治疗等措施后,不良反应发生率明显降低,由36.5%(23/63)降至16.9%(12/71),差异有统计学意义(χ^(2)=6.650,P=0.010)。结论临床药师通过分析那屈肝素钙引起转氨酶升高不良反应的特点,提出相应的药学干预对策,降低了其不良反应的发生率。Objective To reduce the incidence of adverse reactions by finding out the drug that causes the increase of transaminase in patients with gynecological malignant tumors after operation in our hospital,analyzing the characteristics of adverse reactions,and putting forward the countermeasures of pharmaceutical intervention so as.Methods The clinical pharmacists analyzed the association between adverse reactions and drugs as well as its characteristics in 24 patients with elevated transaminase after gynecological tumor surgery in our hospital from September to December,2018.Advice of prevention and treatment were put forward for clinicians.Results The 24 patients were(49.90±7.76)years old,and their body weight was(56.20±7.11)kg.The adverse reaction of one patient was considered to be caused by chemotherapy drugs,and the other 23 cases'were caused by nadroparin calcium.The incidence of adverse reactions was reduced from 36.5%(23/63)to 16.9%(12/71)after adjusting the dosage of nadroparin calcium and the monitoring time of liver function and the timely treatment of liver protection(χ^(2)=6.650,P=0.010).Conclusion Clinical pharmacists reduce the incidence of adverse reactions by analyzing the characteristics of adverse reactions of elevated transaminase induced by nadroparin calcium and putting forward corresponding countermeasures.
