血清缺血修饰白蛋白、可溶性白细胞分化抗原40配体水平与急性冠脉综合征患者预后的相关性

摘　　要：目的探讨血清缺血修饰白蛋白(IMA)、可溶性白细胞分化抗原40配体(sCD40L)水平与急性冠脉综合征(ACS)患者预后的相关性,为改善ACS患者预后的临床诊疗提供指导。方法选择2019年1月至2020年1月就诊于开封市中心医院的75例ACS患者作为研究对象,依据患者预后情况分为良好组、不良组。比较患者基线资料及血清IMA、sCD40L水平表达,分析血清IMA、sCD40L水平表达与ACS患者预后的相关性。结果 75例患者中57例预后良好,18例预后不良。不良组血清IMA、sCD40L水平高于良好组(P<0.05);经Pearson相关性检验结果显示,ACS患者血清IMA与sCD40L水平呈正相关(r=0.790, P<0.01);经Logistic回归分析结果显示,血清IMA、sCD40L水平高表达是ACS患者预后不良的影响因素(O^R>1, P<0.05)。结论血清IMA、sCD40L水平与ACS患者预后密切相关,血清IMA、sCD40L水平高表达会导致ACS患者预后不良,通过持续监测血清IMA、sCD40L水平可预测ACS患者预后情况。【Objective】To investigate the relationship between serum ischemia modified albumin(IMA)and soluble leukocyte differentiation antigen 40 ligand(sCD 40 L)levels and the prognosis of patients with acute coronary syndrome(ACS),to provide guidance for clinical diagnosis and treatment to improve the prognosis of ACS patients.【Methods】Seventy-five ACS patients treated in our hospital from January 2019 to January 2020 were selected as the research objects.According to the prognosis of patients,they were divided into a good group and a poor group.The baseline data and serum IMA and sCD40L level expression of patients were compared,the correlation between the expression of serum IMA and sCD40L and the prognosis of ACS patients was analyzed.【Results】Among 75 patients,57 had a good prognosis and 18 had a poor prognosis.The levels of serum IMA and sCD40L in the poor group were higher than those in the good group,and the difference was statistically significant(P<0.05);the Pearson correlation test showed that serum IMA and sCD40L levels in ACS patients were positively correlated(r=0.790,P<0.01);Logistic regression analysis showed that high levels of serum IMA and sCD 40 L were the influencing factors of the poor prognosis of ACS patients(O^R>1,P<0.05).【Conclusion】Serum IMA and sCD40L levels are closely related to the prognosis of ACS patients.High expression of serum IMA and sCD40L levels can lead to poor prognosis in ACS patients.Continuous monitoring of serum IMA and sCD40L levels can predict the prognosis of ACS patients.
