蒿芩清胆汤治疗社区获得性肺炎(湿热内郁型)的疗效及对血清炎症因子、血气指标的影响

摘　　要：目的:观察蒿芩清胆汤对社区获得性肺炎(湿热内郁型)患者疗效及对血清炎症因子、血气指标的影响。方法:选择2018年1月~2019年2月我院社区获得性肺炎(辨证为湿热内郁型)患者98例,随机分为2组,各49例,对照组采用左氧氟沙星治疗;观察组采用蒿芩清胆汤治疗,每日1剂,2周为1疗程。比较两组间的临床疗效、血清降钙素原(Procalcitonin, PCT)、白介素-1β(Interleukin-1β,IL-1β)、肿瘤坏死因子α(Tumor Necrosis Factor-α,TNF-α)及动脉血二氧化碳分压(Arterial Partial Pressure of Carbon Dioxide, PaCO2)、动脉血氧分压(Arterial Partial Pressure of Oxygen, PaO2)及血氧饱和度(Oxygen Saturation of Blood, SpO2)水平。结果:对照组有效率79.59%低于观察组93.88%,差异有统计学意义(P<0.05)。与对照组比,观察组治疗后血清PCT、IL-1β、TNF-α水平较低,PaCO2较低,PaO2、SpO2较高,差异有统计学意义(P<0.05)。结论:蒿芩清胆汤对社区获得性肺炎(湿热内郁型)患者的疗效显著,可以降低炎症因子,改善血气指标,且安全性高,值得推广。Objective: To investigate the effect of Haoqin Qingdan Decoction for patients with community acquired pneumonia(damp-heat and internal depression type) and its influence on serum inflammatory factors and blood gas indexes. Methods: 98 patients with community-acquired pneumonia(syndrome differentiation as damp-heat and internal depression type) from January 2018 to February 2019 in our hospital were randomly divided into two groups, 49 cases in each group. The controlled group received Levofloxacin treatment, the observation group received more with Haoqin Qingdan Decoction, 1 dose a day, 2 weeks as a course of treatment. The clinical efficacy, serum procalcitonin(PCT), interleukin-1β(IL-1β), tumor necrosis factor-α(TNF-α), arterial partial pressure of carbon dioxide(PaCO2), arterial partial pressure of oxygen(PaO2) were compared between the two groups, PaO2 and oxygen saturation of blood(SpO2). Results: The effective rate of the controlled group(79.59%) was lower than the observation group(93.88%),(P < 0.05). Compared with the controlled group, the levels of serum PCT, IL-1β, TNF-α in the observation group were lower, PaCO2 was lower, PaO2 and SpO2 were higher, the difference were statistically significant(P < 0.05). Conclusion: Haoqin Qingdan Decoction has significant curative effect for patients with community-acquired pneumonia(damp-heat and internal depression type), can reduce inflammatory factors, improve blood gas index, and has high safety, which is worthy of promotion.
