蓉黄益肾解毒汤对CKD-MBD肾虚湿热证患者超敏C反应蛋白、同型半胱氨酸水平的影响

摘　　要：目的:研究蓉黄益肾解毒汤对CKD-MBD肾虚湿热证患者超敏C反应蛋白、同型半胱氨酸水平的调节和钙磷代谢的影响。方法:收集符合病例选择标准的CKD-MBD患者,采用随机数字表法,随机分为治疗组和对照组(按照1:1分配入组)。两组均予以西医基础治疗,治疗组加用蓉黄益肾解毒汤,干预12周。观察治疗前后两组患者中医证候积分变化及肌酐(Scr)、eGFR、血钙(Ca)、血磷(P)、甲状旁腺激素(iPTH)、超敏C反应蛋白(hs-CRP)、同型半胱氨酸(HCY)水平。结果:两组最终各纳入20例患者完成治疗随访。治疗后,治疗组患者中医证候积分值明显改善(P<0.01),并优于同期对照组(P<0.05);治疗组Scr、eGFR均改善显著(P<0.05),而对照组均无明显改善(P>0.05);两组Ca、P、iPTH水平均显著改善(P<0.05);治疗后组间比较除Ca外,治疗组P、iPTH均显著优于对照组,差异均有统计学意义(P<0.05);两组hs-CRP、HCY水平均均显著下降(P<0.05或<0.01),而治疗后治疗组水平均显著低于对照组,差异均有统计学意义(P<0.05)。结论:蓉黄益肾解毒汤对CKD-MBD肾虚湿热证具有显著的治疗作用,并能下调患者血中hs-CRP、Hcy水平,调节钙磷代谢紊乱,保护肾功能。Objective: To study the effect of Ronghuang Yishen Jiedu Decoction on the regulation of high sensitive C-reactive protein, homocysteine and calcium and phosphorus metabolism for CKD-MBD patients with kidney deficiency and damp heat syndrome. Methods: The CKD-MBD patients who met the criteria of case selection were collected from June 2018 to December 2019 in the hospital. They were randomly divided into a treatment group and a controlled group(according to 1:1 allocation). Both groups were treated with basic western medicine. The treatment group was treated with Ronghuang Yishen Jiedu Decoction in addition to the controlled group for 12 weeks. Before and after treatment, the changes of TCM syndrome integral and the levels of Scr, eGFR, Ca, P, PTH, hs-CRP, HCY were observed. Results: Finally, 20 patients were included in the two groups to complete the treatment and follow-up. After treatment, the TCM syndrome score of the treatment group was significantly improved(P < 0.01), which was better than that of the controlled group at the same period(P < 0.05);SCR and EGFR in the treatment group were significantly improved(P < 0.05), but there was no significant improvement in the controlled group(P 0.05);the levels of Ca, P and PTH in the two groups were significantly improved(P < 0.05) After treatment, P and PTH in the treatment group were significantly better than those in the controlled group(P < 0.05);the levels of hs-CRP and Hcy in the treatment group were significantly lower than those in the controlled group(P < 0.05 or <0.01), and the water level in the treatment group was significantly lower than that in the controlled group(P < 0.05). Conclusion: Ronghuang Yishen Jiedu decoction can effectively treat CKD-MBD with kidney deficiency and damp heat syndrome, reduce the level of hs-CRP and Hcy, regulate the disorder of calcium and phosphorus metabolism, and protect the renal function.
